Objective:The purpose of this research is to expound the impact factors of balance function among patients with type 2 diabetes by evaluating their balance function.This study can provides the accuracy of fall risk assessment and the timeliness of detection potential fellers.Combining with the clinical characteristics and effective interventions,this study can provides theoretical basis to improve the balance function of patients and reduce their incidence of falls.Methods:Selected were 294 individuals with type 2 diabetes hospitalized in the endocrinology in the first central hospital of Baoding city from February to July 2016.Their information such as age,gender,height,weight,whether exercise;situation of drug treatment,whether have hypertension,coronary heart disease,osteoporosis,diabetes complications;laboratory results including fasting glucose,glycosylated hemoglobin,total cholesterol,triglycerides,fasting serum insulin and serum 25(OH)D3 was collected.With medical records of examination and diagnosis,Doppler flow measuring instrument was used to measure and calculate the ankle brachial index.Tetrax balance measuring instrument was used to measure the balance function under the eight standing positions and calculate the fall index through the instrument patent algorithm.Independent-Samples t-Test and One-way ANOVA were performed to find the independent variables which significantly correlated to the fall index.With the fall index as the dependent variable,the fall index with the meaningful variables was analyzed via multiple regression analysis.Results:1.The fall index of 294 patients from 1 to 100,average number index is 47.86±8.18,including 128 patients from 1 to 36,average number index is 22.82±9.42,in a low risk to fall,the fall index of 166 patients from 37 to 100,average number index is 67.48±21.57,in a intermediate and high risk to fall.2.Age(t=16.61,P<0.001),degree of education(F=3.169,P=0.025),whether exercise(t=-4.804,P < 0.001),situation of drug treatment(F=5.662,P=0.001),hypertension(t=-2.686,P=0.008),coronary heart disease(t=-1.742,P=0.012),the course of the disease(F=10.325,P<0.001),glycosylated hemoglobin(t=-2.339,P=0.020,diabetic peripheral neuropathy(t=-2.216,P=0.027),diabetic retinopathy(t=-3.446,P=0.001),diabetic nephropathy(t=-3.556,P < 0.001),diabetes legs vascular complication(t=-3.281,P=0.001)were the statistical significance variables by using Independent-Samples t Test and One-way ANOVA analyzed.3.The fall index as the dependent variable,situation of drug treatment,whether exercise,hypertension,degree of education,diabetic retinopathy,diabetic nephropathy,diabetic peripheral neuropathy,diabetes legs vascular complications,age,the course of the disease,coronary heart disease,glycosylated hemoglobin as the independent variables,use multiple regression analysis the fall index with the meaningful variables.Ultimately whether exercise(β=-0.320,P<0.001),age(β=0.220,P<0.001),diabetic retinopathy(β=0.160,P=0.005),diabetic nephropathy(β=0.156,P=0.007),glycosylated hemoglobin(β=0.143,P=0.012),situation of drug treatment(β=0.135,P=0.017)were entered the regression equation.R2 shows the six variables together explained 27.6% variance of the fall index.Factors contribution to the fall index in patients with type 2 diabetes from high to low are whether exercise,age,diabetic retinopathy,diabetic nephropathy,glycosylated hemoglobin,situation of drug treatment.Conclusion:Whether exercise,age,diabetic retinopathy,diabetic nephropathy,glycosylated hemoglobin,situation of drug treatment are the main factors influence balance function of patients with type 2 diabetes.Internal and external factors together causes balance dysfunction in patients with type 2 diabetes.Patients should choose appropriate exercise according to oneself circumstance to improve balance function,positive control of blood sugar,pay attention to safety in the process of drug treatment,actively participate in the treatment to delayed complications,pay attention to the potential risk to prevent fall.
