The Brief Analysis Of Chronic Rhinosinusitis Related Factors

Objective: To investigate relative pathogenic factors of chronic rhinosinusitis(CRS),and analyzing the pathogeny of CRS.Methods: 318 cases patients with CRS,diagnosed by surgery and pathology examination,have been done nasal endoscopic and sinus CT examination before operation.The object of study is divided into two groups of CRSsNP and CRSwNP.Combined with preoperative peripheral blood eosinophilia,serum total IgE,specificity IgE,complement C3,C4 and limuis test,sinus CT scanning,intraoperative germiculture and nasal polyp specimens examination results,etc.Clinical data,analyze the correlation between CRS and three aspects of allergic factors,anatomical factors,infection factors.SPSS20.0 statistical software was used to analyze the data.Results: 1.In 318 cases patients with CRS,patients with allergic factors was 157 cases,accounting for 49.37 %.Among them,the patients with CRSsNP was 22 cases(22.68%),patients with CRSwNP was 135 case(61.09%),chi-square test between them was statistically significant(P < 0.05);2.In 318 cases patients with CRS,patients with nasal sinuses anatomic abnormalities was 195,accounting for 61.35%,and moderately severe nasal septum deviation is the most common,followed by abnormal uncinate process(deviation and hypertrophy),screen bubble hypertrophy,abnormal middle turbinate(gasification and recurvation),maxillary sinus natural hole closure,etc.;3.In 318 cases patients with CRS,peripheral blood C3 and C4 abnormal(higher or lower)respectively were 105 cases(33.02%),111 cases(34.91%).Peripheral blood positive limuis test was 56 cases(17.61%);Bacteria were detected in 132 samples,accounting for 41.51%,and staphylococcus aureus was the most common.Conclusion: CRS is the result of interaction of many factors,the anatomical factors are the most common,followed by allergic factors and infection.Allergic factors play an important role on the occurrence and development of CRS,especially one of the main reasons for the occurrence and recurrence of CRSwNP;The occurrence of CRS is usually associated with the variation of nasal sinus anatomy,and with the severity of the disease increased,the incidence rate of nasal anatomic variation increases,the common nasal anatomic variation were nasal septum deviation(severe),uncinate process abnormalities(deviation and hypertrophy)and middle turbinate gasification;Bacterial infection plays an important role in the pathogenesis of CRS,and the anatomical abnormalities of nasal cavity and paranasal sinuses can increase the probability of pathogen infection.
