AIM To assessment the influences of ulinastatin on renal function in patients with septic shock.METHODS The randomized controlled trials of ulinastatin treatment in patients with septic shock were searched from Cochrane library,PubMed,Google,Ovid,EMbase,Chinese Biological Medicine,China National Knowledge Infrastructure database,VIP and Wanfang database by computer.Data were collected from inception to 2015.Two researchers screened the literatures independently according to inclusion criteria.The Cochrane’s bias risk assessment criteria were used to evaluate the quality of literature.The evaluating indicators included urine volume,N-acetyl-beta-D-amino glycosidase enzymes,gamma-glutamyl transferase,serum creatinine,blood urea nitrogen and the proportion of continuous renal replacement therapy.The Review Manager 5.3 software was used for Meta-analysis.RESULTS Eight randomized controlled trials were included which involving 410 patients.The results of meta-analysis showed that the urine volume in the ulinastatin group was higher than the control group in the treatment of 1,2,3 days.Meanwhile,the N-acetyl-beta-D-amino glycosidase enzymes and gamma-glutamyl transferase in ulinastatin group was lower than the control group in the treatment of 1,3,5 days,the differences were statistically significant(P<0.05).The proportion of continuous renal replacement therapy(RR=0.50,95%CI:0.28?0.91,P=0.02)in ulinastatin group was significantly lower than the control group.CONCLUSION The ulinastatin could improve the renal function of patients with septic shock in partly.However,ti is far from accurate enough to recommend ulinastatin as a conventional therapy for septic shock.Due to the limite of literature quality and sample size.Therefore,more double-blind randomized controlled trials with large sample,high quality are required for further argument.
