The Effecting Of Removal The Intraocular Foreign Bodies By Pars Plana Vitrectomy And Prognosis Analysis

Background: Intraocular foreign bodies is one of the most important element among young adults leading to the loss of vision.We want to according to the clinical features of patients with posterior segment intraocular foreign bodies collected clinical data,analysising of open-ended vision prognosis in patients with ocular trauma intraocular foreign bodies and vitreous cut effect on the prognosis of patients.We open from trauma vitreous cut out patients with intraocular foreign bodies,and analyzes the results of their final vision after intraocular foreign bodies and influencing factors and complications such as multiple related factors.Purpose: Analysis trauma patients with posterior segment foreign body in vitreous cut treatment effect and prognosis of vision related factors.Method: Retrospective analysis between December 2015 and January 2017 in posterior segment intraocular foreign bodies(40 eyes,all in posterior segment intraocular foreign bodies by pars plana vitrectomy).For age,gender,preoperative/postoperative best corrected visual acuity(BCVA),posterior segment foreign material properties,size,intraocular foreign body removal time,position of intraocular foreign bodies,intraocular foreign bodies left position,operation time,operation method and complications such as factors in the statistics.Using SPSS19.0 for data analysis.Results: Patients with an average age of 43.15 years,37 cases(92.5%)of men and women in 3 patients(7.5%),effective treatment of 25 cases(62.5%)patients,16 patients restore vision(40%),effective recovering effective vision 24 cases,and blindness of these patients were 2 cases(5%).Foreign bodies in the vitreous cavity of the 12 cases(30%),embedded in the retina in 18 eyes(45%),penetrating the wall of eyeball in 10 cases(25%).Merging of 10 cases of hyphema eyes(25%),combination of vitreous hemorrhage in 24 cases(60%),merging of retinal detachment in 19 eyes(47.5%),merging the retina injury in 28 cases(70%),merging endophthalmitis has 8 cases(20%).The diameter of foreign body less than 3 mm has 18 cases(45%),being equal or greater than 3 mm are 22 cases(55%).Intraocular foreign bodies in two days out of 26 cases(65%),2 days out of 14 cases(35%).Preoperative best corrected visual acuity 4.0 are 8 eyes(20%)cases,preoperative best corrected vision less than 4.0 has 32 eyes(80%).Vision in patients with the main influence factors for retinal detachment(P = 0.000),the retina injury(P = 0.001),the size of the intraocular foreign bodies(P = 0.023),the time of removing the foreign bodies(P = 0.006),hyphema(P = 0.032),vitreous hemorrhage(P = 0.025),and preoperative BCVA(P = 0.014).Endophthalmitis has no obvious correlation with the prognosis of visual acuity(r = 0.089,P > 0.089).The length of the posterior segment foreign body are negatively correlated with preoperative and the postoperative best corrected visual acuity(P < 0.05).Retinal detachment is associated with the final best corrected visual acuity(BCVA)(P < 0.05).The retina injury is associated with postoperative BCVA(P < 0.05).Removing foreign bodies time also is associated with vision prognosis(P < 0.05),the sooner taking out the foreign body,the better of postoperative BCVA.Hyphema and vitreous hemorrhage also influence postoperative BCVA(P < 0.05),and to clear the hyphema and vitreous hemorrhage can improve postoperative BCVA.The better of the preoperative BCVA,postoperative BCVA will be better(P < 0.05).There is no obvious correlation of the intraocular foreign bodies removal time have direct contact with the incidence of endophthalmitis(r = 0.183,P > 0.183),and endophthalmitis basic don’t affect vision.Conclusion: Greater intraocular foreign bodies and combination of retinal detachment,retinal hiatus,hyphema,vitreous hemorrhage,the time of removing foreign bodies is longer in the open in trauma can cause more serious consequences,eyesight will be worse prognosis.Goggles to prevent injury plays a significant role.In our study,whether the suture of Ⅰ period and the time of vitreous cutting had no significant effect on the recovery of vision and anatomy.Therefore,we can advance the suture of Ⅰ period and glass cavity medicine injection,vitrectomy can postpone to 23-48 hours,but we have to removal the IOFBs as soon as possible.
