Relationship Between The Plasma Copeptin Levels And Left Ventricular Remodeling And Cardiac Function In Patients With Chronic Heart Failure

Objective:To investigate the changes between the plasma copeptin in patients with chronic heart failure(CHF),and analyze the correlation of plasma copeptin,ventricular remodeling and cardiac function.Methods:A total of 100 patients with CHF from The Cerebro-Cardiovascular Disease Hospital of Ningxia Medical University were included in this study(CHF group),and 30 healthy persons were used as control group.All patients with heart failure were in accordance with the diagnostic criteria of the 2014 Chinese diagnostic and therapeutic guidelines for heart failure.in accordance with the NYHA class the CHF group were divided into grade II 35 cases,grade III 32 cases,grade IV 33 cases.And CHF group was also divided into heart failure with reduced left ventricular ejection fraction(HFr EF,LVEF<40%)group(n=52)and heart failure with preserved left ventricular ejection fraction(HFp EF,LVEF? 40%)group(n=48),groups were in accordance with LVEF level of patients.The plasma copeptin and brain natriuretic peptide(BNP)levels were detected by ELISA.The values of LVEF,left ventricular end-diastolic diameter(LVDd),left ventricular systolic diameter(LVDs)and left ventricular fractional shortening(LVFS)were detected by echocardiography.The correlation of copeptin,NYHA classification,BNP and index of echocardiography was analyzed between groups.Results:(1)Compared with control group,the plasma copeptin was significantly higher in people with heart failure.And the copeptin level increased gradually with the deterioration of cardiac function,the difference is significant.(P<0.05)(2)The levels of BNP,copeptin were significantly higher in HFr EF group than in HFp EF group(P < 0.05).(3)Compared with the control group,the plasma levels of copeptin,BNP and the value of LVDd?LVDs in heart failure patients were significantly higher,EF,FS were significantly lower.(P<0.05)(4).Using Pearson correlation analysis,the plasma copeptin was positively correlated with BNP? LVDD and LVDS(r=0.58 ? 0.63? 0.65,P<0.01),and was negatively correlated with left ventricular ejection fraction(EF)and left ventricular fractional shortening(FS)(r=-0.68 ?-0.69,P<0.01).The ROCs are copeptin 0.977(95% CI 0.499-0.705);BNP 0.976(95% CI 0.954-0.999).And the best critical points are BNP 144.5pg/ml,copeptin 15.595pmol/L Conclusion:(1)The level of plasma copeptin was significantly increased in patients with chronic heart failure.(2)The level of plasma copeptin was correlated with the parameters of left ventricular remodeling in patients with heart failure.(3)The level of plasma copeptin was significantly correlated with the severity of heart failure.(4)The plasma copeptin has potential clinical application value in heart failure patients.
