Objectives: To investigate the value of liquid-based cytology in the diagnosis of lung cancer,and to analyze the diagnostic value of liquid-based cytology combined with other methods.Methods: Total of 160 patients,who have lesion in our department between October2016-February 2017,examined by fiberoptic bronchoscopy with forceps biopsy,thinprep cytology test,brush smears and DNA ploidy analysis.There were 88 lung cancer patients(Group A,88 cases)and 72 non-lung cancer patients(Group B,72 cases).To compare forceps biopsy,brush smears,thinprep cytology test and DNA ploidy analysis individually and combined for diagnosis of lung cancer.Data analyses were performed by using the SPSS software package version 21.0.Statistical methods used chisquare test.All statistical analyzes were a=0.05 level for the test standard,P<0.05 was considered to be statistically significant.Results:1.To compare the sensitivity:Forceps biopsy(72.73%)> DNA ploidy analysis(71.59%)> LBC(67.04%)> CS(52.27%);To compare the specificity: Forceps biopsy(100%)> LBC(98.61%)> CS(91.67%)> DNA ploidy analysis(84.72%).LBC compared with CS,the difference of sensitivity was statistically significant(P<0.05),but the difference of Specificity was no statistically significant(P>0.05).Analysis of the effectiveness of individual method:The Kappa value of forceps biopsy,LBC,DNA ploidy analysis and CS were 0.706,0.634,0.553,0.420.The sensitivity,Specificity,Youden index,positive predictive value,negative predictive value,Kappa value and accuracy of LBC were higher than the method of CS.2.The Specificity of LBC+forceps biopsy was the highest(98.61%).Analysis of the effectiveness of joint method: LBC+forceps biopsy,LBC+CS+forceps biopsy,LBC+DNA ploidy analysis+forceps biopsy,LBC+CS+DNA ploidy analysis+forceps biopsy,LBC+CS+DNA ploidy analysis compared with the clinical diagnosis of highly consistency(Kappa value was 0.789,0.725,0.709,0.708,0.673).The Specificity,Youden index,positive predictive value,negative predictive value,Kappa value and accuracy of LBC+forceps biopsy were the highest.Conclusions: LBC compared with the pathology diagnosis of highly consistency.The sensitivity and Specificity of combined detection can significantly improve than the single detection,which of the LBC+forceps biopsy was best.LBC for auxiliary diagnosis of lung cancer has more value than CS. |
