The Use Of Liquid Chip Technology In The Application Of Acute Lower Respiratory Virus Infection And Clinical Characteristics Analysis

Objective:The detection technology of liquid chip is used to test the nucleic acid of sputum/BALF samples from the patients with acute lower respirarory tract infection,to investigate the spectrum of virus,in order to evaluate the clinical value of liquid chip detection technology and analyze the distribution of viral etigology,and Collect the clinical information of selected cases,and Analysis the epidemiology and clinical features of acute lower respiratory virus infection,hope to be helpfor the clinical diagnosis and treatment.Methods:327 cases of lower respirarory tract infection in Fujian Provincial Hospital are collected from January 2015 to May 2016.The detection technology of liquid chip established by our research group is used to detect the nucleic acid of sputum/BALF samples.It can detect 12 kind of virus.Then the spectrum is analyzed,compared with Real-Time PCR test results,to evaluate the positive coincidence rate 、 negative coincidence rate、the total coincidence rate and Kappa value.According to the result of golden criterion,the pathogen specific cases can be divided into viruses and bacteria group,and collect their clinical information,analysis the clinical features of bacterial infection spread of acute lower respiratory infection with the chi-square test,methods of linear analysis and multi-factor logistics analysis.Results:Of the 327 sputums/BALF detected by liquid chip technology,126(38.5%)cases were positive,and100 cases(79.4%)were only a single virus infection among it,the others were 2 kinds of virus infection and give priority to with mixed cytomegalovirus and Influenza A virus.A total of 152 strains of virus were detected,Influenza A virus(43.4%)and metapneumovirus(40.8%)were the most.The RT-PCR technique was used as the gold standard to evaluate the liquid chip technology,it showed no significant difference(P > 0.05)with the virus detection results and the total detection rate.The sensitivity was 69.6%,the specificitywas 98.3%,and the Kappa value was 0.641.It seems that this 2 kinds of testing result has a high consistency.Compared of viruses and bacteria group with their clinical features,it found that the time from onset to see a doctor、one more basic diseases、fever(37.3℃)or higher,especially the temperature is greater than 38.5℃,no sputum/white and white blood cells normal were the specific clinical features of viral infection.Conclusions:The liqid chip technology can test 12 kinds of virus the same time.Compared with the RT-PCR detection,it had a high consistency,and it can be used as a screening tool to virus infection.Acute lower respiratory infection is given priority with single virus infection,and influenza a virus and cytomegalovirus were the most.Tt found that the time from onset to see a doctor、one more basic diseases、fever(37.3℃)or higher,especially the temperature is greater than 38.5℃,no sputum/white and white blood cells normal were the specific clinical features of viral infection,and it canbe used as a reference for clinical diagnosis and treatment.
