Case Reports Of Prosthodontic Treatment Of Temporomandibular Joint Disorders

Objective: this research was to evaluate the clinical effects of different occlusal splints in the treatment of different types of temporomandibular joint disorder(TMD).Methods: Two patients were included from December 2015 to March 2017 at Fujian Stomatological Hospital.Accroding to clinical symptoms,and CBCT examinations,select one patient of anterior disc displacement with reduction of the temporomandibular joint,and one patient of acute anterior TMJ disc displacement without reduction.Reversible anterior disc displacement of Temporomandibular joint using repositioning splints for treatment,the splint is a physician referral for silent movement and forward movement,find clicking of joint symptoms and degree of mandibular advancement splints made of at the minimum.In patients with acute anterior disc displacement irreversibility manipulative reduction first disc reset,reset key using improved pivot splints,the splint was made on the basis of stabilization splint and the occlusal contact area lies only at the last molar.Both cases were used splints at first and then used occlusal reconstruction approach to maintain the relationship between articular disc and condylar.Record fricton index before and after treatment in patients with temporomandibular joint disorder,CBCT position of the temporomandibular joint changes were observed and compared,explore the splints to treat Temporomandibular joint disorders clinical efficacy.Results:1.Friction index After treatment of the two patients,the temporomandibular joint disorders index decreased.2.Imaging examination CBCT examination before and after the treatment,patients with bilateral CBCT articular disc and condylar relations basic returned to normal.Conclusion:1.After treatment,patients with joint clicks disappeared,joint muscle symptoms relief,and articular disc and condylar relationship adjustment,the functional movement of mandibular was coordinated.The repositioning splint was effective in treating patients of anterior disc displacement with reduction.2.The improved pivot biteplate more easy compared to traditional pivot biteplate,for its higher adaptability with patients.The bite plate by using the principle of leverage to adjust the relationship of condylar articular disc,is an effective method in treating irreversible articular disc displacement.
