ObjectiveThrough the investigation o find the occupational health of the companies using DMF in some enterprises in a certain county level city of Fujian and the health damage of workers exposed to DMF,the study was conducted to human health impacts caused by occupational exposed to DMF,to explore the low concentrations of DMF on the liver damage in workers.MethodsUsing cross-sectional study,current status of occupational hazards in 24 enterprises and occupational physical examination data of 1,122 workers was collected according to questionnaire survey,field test,and occupational physical examination.Analysis of health damage to the working population was then performed to identify potential relevant influential factors in combination with analysis of normality test,rank sum test,variance,chi-square test,and non-conditional logistic regression analysis.Results1.Mean concentration(95%CI)of DMF was 24.6(20.9,28.7)mg/m3 detected in 254 testing points of the 24 enterprises,among which there were 97 testing points with DMF concentration exceeding the occupational exposure limits,with the over-standard rate of 38.2%;furthermore,there were significant differences of DMF concentration in the air of operating posts,where the concentration in the weighing and coating was evidently higher than that in other operating posts.the difference was not statistically significant of DMF concentration in the air of different workshops(P <0.05).2.Exposed group compared with the control group,ALT abnormality was significantly higher than that in the control group,the difference was statistically significant(P <0.05),There were no statistical differences of hepatomegaly,fatty liver,splenomegaly,renal injury,electrocardiographic abnormality and hypertension between exposed group and control group.3.Multivariate logistic regression analysis results of health impacts and influential factors:(1)High concentration exposure of DMF,overweight and obese and man were influential factors of abnormal liver function,OR(95%CI)were 2.491(1.303,4.763)、2.575(1.783,3.720)and 8.041(1.913,33.806),respectively;(2)Good individual protection was a protective factor for the occurrence of fatty liver,OR(95%CI)was 0.279(0.145,0.537),whereas overweight or obese was risk factors for the occurrence of fatty liver,OR(95%CI)was 3.667(2.430,5.535);(3)Good individual protection was a protective factor for splenomegaly,OR(95%CI)was 0.391(0.200,0.762);(4)High concentration exposure of DMF was a risk factor of renal injury,OR(95%CI)was 2.444(1.112,5.375),man was a protective factor for renal injury,OR(95%CI)was 0.288(0.127,0.655);4.Abnormal rate of ALT was significantly higher in the low concentration exposure of DMF group than in control group(P<0.05).While there was no statistical difference of hepatomegaly or fatty liver positive rates between the exposed group and control group(P >0.05).5.Multivariate logistic regression analysis results of liver damage in low concentration exposure of DMF population and influential factors:(1)Overweight or obese was risk factor of abnormal liver function,OR(95%CI)was 3.068(1.726,5.452);(2)Good individual protection was a protective factor for the occurrence of fatty liver,OR(95%CI)was 0.291(0.138,0.611),overweight or obese and older age were risk factors for the occurrence of fatty liver,OR(95%CI)were 3.469(2.154,5.587)and 2.262(1.095,4.670),respectively.Conclusion1.The concentration of DMF in the air is relatively higher in enterprises with the application of DMF in this region,particularly in operating posts including weighing and coating which went beyond the standard greatly.2.Workers exposed to DMF can cause liver damage,splenomegaly and renal injury,but without obvious influence in electrocardiogram and hypertension,meanwhile,Workers exposed to concentrations of DMF less than the occupational exposure limit still have the possibility of liver damage.Strengthening the prevention and treatment of toxic liver disease,toxic nephropathy.3.Concentration exposure of DMF and individual protection were influential factors of workers health.
