Objective:Study of tranexamic acid by intravenous and intra articular injection of hemostatic efficacy and safety of the difference in total knee replacement surgery.Methods:The use of Cochrane database,PUBMED database,CNKI China China biomedical journal full-text database and document database,Wanfang database,supplemented by manual retrieval,literature review of related literature at home and abroad,collecting2011-01/2016-10 official journal published on the total knee replacement surgery to stop bleeding in English control study literature through two ways and intra-articular injection intravenous injection of tranexamic acid,selected all references included in the standard and in strict accordance with the quality of the included studies and extracted relevant data,and using Rev Man5.0 software statistical analysis of all relevant data.Results:Results a total of 6 articles were included,578 patients were included,of which the local application of intra-articular injection of TXA288 cases,intravenous injection of TXA.290 cases of articular cavity injection and intravenous injection of TXA in preoperative hemoglobin [MD=0.08,95Cl%(-0.09,0.24)P=0.36],postoperative hemoglobin [MD=0.10,Cl95%(-0.12,0.31)P=0.38],preoperative hematocrit the amount of [MD=-0.34,Cl95%(-1.23,0.54)P=0.45],postoperative hematocrit [MD=-0.09,Cl95%(-2.07,1.90)P=0.93],the total blood loss was [MD=0.93,Cl95%(0.21,4.16)[MD=-0.45 P=0.92] blood transfusion rate after surgery,Cl95%(-1.20,0.29)P= 0.23],days of hospitalization [MD=1.50,Cl95%(0.62,3.66)[MD=1.50 P=0.37] and thromboembolic events,Cl95%(0.62,3.66)P=0.37] showed no significant differences.Conclusion:Conclusion there is no difference between the efficacy and safety of intraarticular injection and intravenous injection of tranexamic acid to reduce bleeding in total knee replacement surgery,two medication can reduce postoperative bleeding after TKA and two routes of administration in deep vein thrombosis and pulmonary embolism has no obvious difference and rate.
