Objective1.To analyze the general situation in patients with SLE,IL-2 treatment group and healthy controls(HC).2.To compare the expression of Th cells,Th17 cells and Treg cells in SLE patients with different disease activity.3.Discussion the expression of Th17 cells and Treg cells in patients with SLE correlation with SLEDAI score,disease course,ESR,C3 and C4.4.Comparison of SLE patients changes of Th17 cells,Treg cells,SLEDAI score,ESR,C3 and C4 before and after low-dose IL-2 treatment.Methods:A total of one hundred and seventy-one cases were included in this study,comprising 128 SLE patients(92.18% female,mean age ± SD 36.53 ± 14.33),43 cases of age and gender matched healthy control group(93.02% female,mean age ± SD 39.30 ± 13.28),of which SLE patients according to the disease activity is divided into three group,including 19 cases of remission stage patients(94.74% female,mean age ± SD 34.42 ± 12.86),low activity group 45 patients(93.3% female,mean age ± SD 39.87 ± 14.72),high activity group 64 patients(90.63% female,mean age ± SD 34.82 ± 14.24).The 128 patients with SLE fulfilling the revised in 1997 by American College of Rheumatology(ACR)criteria(Table 1)for SLE who were admitted to the Second Hospital of Shanxi Medical University from January 2016 to January 2017.According to the SLE disease activity index 2000(SLEDAI 2k)(Table 2),128 SLE patients were divided into remission group(0≤ SLEDAI≤4,n=19),low activity group(5≤SLEDAI≤9,n=45),high activity group(SLEDAI≥10,n=64).Among them,76 patients with SLE(93.42% female,mean age ±SD 38.30 ±14.74),low-dose IL-2(500 thousand units per day)was given for 5 days.The extraction of peripheral blood lymphocytes of subjects,the analysis of expression of Th cells,Th17 cells and Treg cells in SLE patients with different disease activity and healthy controls,the correlation of SLE group Th17 cells,Treg cells and SLEDAI score,disease course,ESR,C3 and C4.Comparison of 76 SLE patients changes of Th17 cells,Treg cells,SLEDAI score,ESR,C3 and C4 before and after low-dose IL-2 treatment.All the data were analyzed by SPSS 17.0.Results1.Sex ratio and age difference in patients with SLE,IL-2 treatment group and healthy control(HC)group had no statistical significance.SLEDAI score in SLE group and IL-2 treatment group had no statistical significance(P>0.05).SLEDAI score between different activities SLE group had statistically significance(P<0.001).2.The count of Th cells in SLE group compared with the HC group significantly decreased(P<0.001);the count of Th17 cells compared with HC group had no statistical significance;the count of Treg cells compared with the HC group significantly decreased(P<0.001),the ratio of Th17/Treg in SLE group compared with HC group significantly increased.The count of Th cells between the different activities SLE group had statistically significance.The count of Th cells in remission group was higher than low activity group(P=0.005).The count of Th cells in remission group was significantly higher than high activity group(P<0.001).The count of Th cells in low activity group was higher than high activity group(P=0.015).The count of Th17 cells between the different activities SLE group had no stastically significance.The count of Treg cells between the different activities SLE group had statistically significance.The count of Treg cells in remission group was higher than low activity group(P=0.001).The count of Treg cells in remission group was significantly higher than high activity group(P<0.001).The count of Treg cells in low activity group was higher than high activity group(P=0.046).The ratio of Th17/Treg in SLE remission group was lower than high activity group(P=0.006).3.The count of Treg cells was negatively correlated with SLEDAI score and ESR(r=-0.329,P<0.001;r=-0.252,P=0.005).The count of Treg cells was positively correlated with C3,C4(r=0.233,P=0.012;r=0.242,P=0.009).The ratio of Th17/Treg was positively correlated with SLEDAI(r=0.271,P=0.002).4.In 76 patients with SLE,Low-dose IL-2 treatment compared with before treatment,the count of Th17 cells significantly increased(P<0.001),the count of Treg cells significantly increased(P<0.001),the ratio of Th17/Treg significantly decreased(P<0.001),the SLEDAI score significantly decreased(P<0.001),the ESR(mm/h)significantly decreased(P<0.001).Conclusion:1.Treg cells play important roles in the pathogenesis of SLE,and are negative correlation with SLE.2.The balance of Th17/Treg cells play vital roles in the pathogenesis of SLE,and its ratio is positive associated with SLE disease activity.3.Low-dose IL-2 therapy can significantly raise the count of Treg cells,and restore the balance of Th17/Treg cells,and then reduce the disease activity of SLE.
