| ObjectiveCerebral infarction is one of the major diseases that seriously endanger human health and life,the administration of rt-PA thrombolytic therapy within 4.5 hours after onset is currently the most effective treatment.However,thrombolytic therapy is a double-edged sword,hemorrhagic transformation is the most serious complication of cerebral infarction after thrombolysis.The mechanism of hemorrhage after thrombolysis is still unclear,the theory of blood-brain barrier damage is widely concerned.It has been suggested that caveolin-1,MMP-9 are associated with the destruction of blood-brain barrier integrity.However,make caveolin-1,MMP-9 as biological markers to explore their relationships with post-thrombolytic prognosis in patients with cerebral infarction are even a blank.The purpose of this study is to detect the level of plasma caveolin-1,MMP-9 in thrombolytic patients with cerebral infarction,to investigate the relationship between caveolin-1,MMP-9and thrombolytic hemorrhage,to make an early prediction of thrombolytic hemorrhagic transformation,and also reduce the risk of hemorrhage after thrombolysis.MethodsForty patients with acute cerebral infarction who underwent thrombolytic therapy were included in this study.Among them,3 cases were enrolled as hemorrhagic transformation group and 37 cases as controls.Plasma samples and the relevant basic clinical data were collected before the patients received thrombolysis therapy.Plasma caveolin-1,MMP-9levels were tested by enzyme-linked immunosorbent assay(ELISA).The comparison between the two proups of data was analyzed using the statistical software spss18.0.ResultsPlasma caveolin-1,MMP-9 levels in patients with cerebral hemorrhage afterthrombolysis increased compared with controls.Patients with poor prognosis had higher caveolin-1 levels compared with controls.There was a positive correlation between levels of plasma caveolin-1 and MMP-9 in all patients.The age,blood glucose,caveolin-1 and MMP-9 levels of patients had a certain effect on thrombolytic hemorrhage.Age,smoking history,atrial fibrillation,NIHSS score before thrombolysis,plasma caveolin-1 concentration had a certain effect on thrombolytic prognosis.ConclusionThe expression of caveolin-1,MMP-9 increased in patients with hemorrhage after thrombolysis in cerebral infarction.Plasma caveolin-1,MMP-9 levels were risk factors for thrombolytic hemorrhage in patients with cerebral infarction and also associated with the prognosis.Caveolin-1,MMP-9 may be useful indicators for the danger of hemorrhagic transformation in thrombolytic patients with cerebral infarction and may be new targets for the treatment of hemorrhagic transformation after thrombolysis. |
