| Objective: To investigate the application value of ultrasound with diffuse optical imaging system on differentiating disease degree of breast lesion.DOT was used to provide saturation of blood oxygen.Application value of nutrition metabolism in differentiating malignant from benign on breast lesion was explored.Methods: Collected 98 cases including 102 breast lesions from June 2015 to January 2017,who examined ultrasound with diffuse optical imaging and have coarse needle biopsy or surgery in the department of thyroid breast surgery in Qinghai University Affiliated Hospital.To compare the pathological results,and analysis of quantitative indicators(HBT,SO2,SDI),evaluated the sensitivity,specific,the rate of accuracy,positive predictive value,negative predictive value of ultrasound with diffuse optical imaging system.The malignant lesions were divided into two groups with the largest internal diameter of 1cm,compared the relationship between the size of tumor and the hemoglobin and blood oxygen saturation in the lesions.According to the pathological results,the breast tumors were divided into benign and malignant groups.Compared the parameters of the two groups(total hemoglobin,blood oxygen saturation)between the benign and malignant tumors.According to the pathological results,the breast neoplasms were divided into benign and malignant groups.The parameters of the two groups(total hemoglobin,blood oxygen saturation)were compared,and studied the difference of the parameters between the benign and malignant neoplasms.Draw the receiver-operating characteristic(ROC)curve,calculating the area under the curve,in order to explicit the value of differentiating malignant from benign breast lesion with indexes(HBT,SO2,SDI).Results: 1.102 cases of breast masses,contained malignant breast masses of 52,50 cases of benign lesions.There were 62 malignant breast tumors and 40 benign tumors were detected by ultrasound with diffuse optical imaging system.The sensitivity of the diagnosis of breast masses by ultrasonic light scattering was 94.23%,specificity was 74%,the rate of accuracy was 84.31%,positive predictive value was 79.03%,negative predictive value was 92.5%.2.The malignant lesions were divided into two groups with the largest internal diameter of 1cm,the maximum diameter of less and equal to 1cm tumor in hemoglobin: 186.67 ± 75.57umol/L,lower than the maximum diameter greater than 1cm tumor in hemoglobin:241.59 ± 57.72umol/L,and the difference was statistically significant(P<0.05).the maximum diameter of less and equal to 1cm tumor in blood oxygen saturation: 1.05 ± 0.25,higher than the maximum diameter greater than 1cm tumor in blood oxygen saturation:0.87 ± 0.20,and the difference was statistically significant(P<0.05).3.The hemoglobin value of the benign tumor was 155.16±49.27,and the hemoglobin value of the malignant tumor was 225.75±67.46,and the difference was statistically significant(P<0.05).The value of blood oxygen saturation of the benign tumor was 1.23±0.30,and the value of blood oxygen saturation of malignant tumor was 0.93±0.23,the difference was statistically significant(P<0.05).4.The ROC curve was drawn by hemoglobin,which area under the ROC curve is 0.826,the best threshold is 205.5umol/L,the sum of corresponding sensitivity and specificity is the largest and its specificity is 90%,sensitivity is 71.2%.The ROC curve was drawn by the oxygen saturation,and the area under the ROC curve was 0.784.The best threshold was 1.075,and the sum of corresponding sensitivity and specificity is the largest,its specificity was 78.8%,the sensitivity was 66%.The ROC curve was drawn with the comprehensive diagnostic index as the index,the area under the ROC curve was 0.918,the best threshold value was 133.5,and the sum of corresponding sensitivity and specificity is the largest,and its specificity was 70%,the sensitivity was about 100%.Conclusion: 1.Ultrasound with diffuse optical imaging system differentiating malignant from benign breast lesion with high clinical value is worthy of application.2.Quantitative indicators(HBT,SO2,SDI)has direct correlation with ofbenign and malignant tumor,can provide more reference for clinical diagnosis and treatment.3.The value of comprehensive diagnostic index in diagnosing benign and malignant breast tumors is obviously higher than that of single index(HBT,SO2),It is helpful for screening breast cancer patients. |
