Objective: The proportion of primary hypertension complicated with type 2 diabetes mellitus was higher.High blood pressure and diabetes can cause multiple target organ damage,particularly increasing the risk of cardiovascular,cerebrovascular,and thromboembolic events.Platelet abnormalities,coagulation,fibrinolytic system changes can lead to the formation of pre-thrombotic state,thus increasing the risk of cardiovascular,cerebrovascular,and thromboembolic events.The aim of this study was to investigate the relationship between platelet parameters and coagulation index,and their clinical significance in hypertensive patients with type 2 diabetes mellitus,and also to investigate the association of vasuclar endothelial function damage with coagulation by detection of levels of AT-Ⅲ,D-D,and VWF.Methods: Hypertension with T2 DM patients(hypertension T2 DM group,n=50)and non-diabetic patients with essential hypertension(hypertension group,n=50)who were admitted to the general hospital of north China petroleum administration bureau in renqiu from January 2016 to October 2016 were enrolled in this study.25 case of healthy people were selected as the control group in the same period.The platelet parameters such as PLT,MPV,PDW and P-LCR were detected by SYSMEX five-factor hematology analyzer.The STAGO COMPACT automatic hemagglutination analyzer was used to analyze the expression of PT,APTT,TT,FIB and other coagulation related indicators;using chromogenic substrate method to detect AT-Ⅲlevels,using immunoturbidimetric to detect D-D,VWF levels.Results:1 There was no significant difference in gender,age,smoking population between 3 groups(P>0.05);compared with the control group,SBP,DBP,MABP,pulse pressure and BMI of hypertensive group and hypertensive T2 DM group were significantly increased(P<0.01),but there was no significant difference between 2 groups(P>0.05).2 Compared with the control group,FPG,2hPG,HbAlc,TC,TG and LDL-C in hypertension group and hypertensive T2 DM group were significantly increased and HDL-C was significantly decreased(P<0.05 or P<0.01).Compared with hypertension group,FPG,2hPG,HbAlc,TC,TG,LDL-C in in hypertension T2 DM group were significantly increased,HDL-C was significantly lower(P<0.05 or P<0.01).3 Compared with the control group,PLT,MPV,PDW,P-LCR in hypertension group and hypertensive T2 DM group were significantly higher(P<0.01);Compared with hypertensive group,PLT,MPV,PDW and P-LCR in hypertensive T2 DM group were significantly higher(P<0.01).4 Compared with the control group,PT,APTT in hypertension group and in hypertension T2 DM group decreased significantly,FIB increased significantly(P<0.01),compared with hypertension group,PT,APTT in hypertension T2 DM group decreased significantly,FIB increased significantly(P<0.01),there was no significant difference of TT between 3 groups(P>0.05).5 Compared with the control group,the levels of AT and AT-Ⅲwere signifi-cantly decreased in hypertensive group and hypertension T2 DM group(P <0.01).Compared with hypertensive group,the AT-Ⅲin hypertension T2 DM group was significantly decreased,D-D and VWF were significantly increased(P<0.05 or P<0.01).Conclusions:1)Platelet parameters and coagulation test can be sensitive to reflect the primary hypertension and T2 DM patients with coagulation function and whether there is abnormal.2)The routine blood parameters and blood coagulation 4 items are the sensitive indexes to reflect the coagulation status of patients with essential hypertension and T2 DM.3)Vascular endothelial cell injury and blood hypercoagulable state are involved in patients with essential hypertension and T2 DM.Detection of AT-III,D-D,VWF,and other molecular markers of PTS can provide a basis for the prevention of thromboembolic events in patients with hypertension and T2 DM.
