| Objective To evaluate the values of the fractional exhaled nitric oxide(Fe NO)and peripheral blood eosinophilia(EOS%)in bronchial asthma diagnosis and assess the state of the illness,and to achieve their best thresholds.Methods From December 2015 to June 2016,in the Department of Respiratory of the First Affiliated Hospital of the Medical College of shihezi University,222 patients with suspected bronchial asthma including wheezing,cough and breathlessness from Respiratory Clinic were selected.All patients were tested Fe NO measurement,pulmonary functions and peripheral eosinophil determination.Among 222 patients,139 cases were diagnosed asthma.The patients of diagnosed asthma were tested by the ACT.The remained 83 cases were diagnosed as non-asthma.Through the statistic,to analyze the related date.Results 1.Asthma groups and non-asthma groups general information were compared display: asthma group aged 18-70(43.8 ± 1.1),59 cases of men,who accounted for 42.4%;80 cases of females,who accounted for 57.6%.Non-asthma groups ged 18-70(46.8 ± 1.5),25 cases of men,who accounted for30.1%;58 cases of females,who accounted for 69.9%.The two groups in gender(male,female),age(year),nation(han,other),BMI(kg/m2),the differences were not statistically significant(P>0.05).2.(1)Fe NO level in asthma group was higher than in non-asthma group[(59.27±3.08ppb)vs(19.65±2.13ppb),P<0.05].FEV1%level in asthma group was lesser in non-asthma group[(75.20±1.59%)vs(92.59±1.34%),p<0.05].EOS%level in asthma group was higher in non-asthma group[(6.583±0.432%)vs(2.282±0.192%),p<0.05].(2)Fe NO in asthma group had no relationship with FEV1%(r=-0.037,p=0.665),and had positive relationship with EOS%(r=0.522,p<0.05).(3)ACT scores in asthma group had negative relationships with Fe NO and EOS%(r=-0.418,r=-0.396,p<0.05).3.The area under ROC curve of Fe NO diagnosing asthma was0.896,95%CI(0.853-0.940),the best cut-off point was 29.5ppb.The sensitivity of Fe NO=29.5ppb diagnosing asthma was 78.4%,specificity 91.6%.The area under ROC curve of EOS% diagnosing asthma was0.783,95%CI(0.722-0.844),the best cut-off point was 3.72%.The sensitivity EOS% diagnosing asthma was70.5%,specificity 86.7%.Conclusion Fe NO and EOS% are important indicators of diagnosing asthma.Fe NO combines with EOS% have more important clinical significance for the diagnosis of asthma.The higher the numerical of peripheral blood eosinophilia and Fe NO,the severity of asthma may be greater. |
