Objective : To evaluate the clinical efficacy of fistula repair by stapler technique in the patients with cervical tracheoesophageal fistula. Methods:Retrospectively analysis the date of 8patients with cervical tracheoesophageal fistula who accept operative treatment in the Department of Thoracic Surgery of Second Hospital of Lanzhou University from October 2014 to October2016,with the male to female ration of 5:3.Among these tracheoesophageal fistula patients,4cases were induced by tracheal intubation,2 cases by esophageal foreign bodies,1 case by tracheal stent, and 1 case by esophageal diverticulum;they all had fistula closed by Stapler technique, and the surgical effects were evaluated through KPS scoring, imaging improvement situation assessment, patient satisfaction investigation and feeding-induced bucking improvement assessment. Results: The operations were performed successfully, operative time was(117.5±6.6)min,introperative blood loss was(60±7.0)mL.After the operations, the patients did not suffer incision bleeding and infection, hoarseness and dyspnea,drinking-induced bucking, fistula relapse, trachea esophagus stenosis or any other complications, and no death occurred during the perioperative period. The chest x-ray test performed 1 week later showed that the pulmonary infection had disappeared, and only 1 patient suffered from esophageal stricture 1 year later. The postoperative KPS score was(90±7.0)points,significantly improved in contrast to preoperation(p<0.01),postoperative pulmonary infection area reduced significantly(p<0.05),tracheoesophageal fistula disappeared, postoperative patients satisfaction survey rated the satisfaction rate was90%,feeding-induced bucking improvement situation assessment was first-rate.Conclusion:Repair cervical tracheoesophageal fistula using stapler method is a safe, easy and useful procedure, what more, the procedure has characteristic of shorter time and less postoperative complications. |
