| Objective: To investigate and analyze the type of pathogens of pneumoconiosis complicated with pulmonary infection and the characteristics of the pathogens drug resistance in Zibo area, and to provide experimental evidence for clinical rational use of antibiotics to prevent infection. Methods: From January of 2011 to January of 2015,a total of 1403 patients with pneumoconiosis were admitted to our hospital.Among them, there were 744 cases of pneumoconiosis complicated with pulmonary infection: 372 cases were in the first phase of the pneumoconiosis and the age of the patients was 42 to 62 years old,223 cases were in the second phase of the pneumoconiosis and the age of the patients was 45 to 79 years old,and 149 cases were in the third phase of the pneumoconiosis and the age of the patients was 44 to 84 years old.The diagnosis of pneumoconiosis is based on the common diagnosis of pneumoconiosis diagnosis expert group of Zibo occupational disease prevention and treatment institute according to the diagnostic criteria of pneumoconiosis(GBZ70-2009).We Collected the sputum samples of the patients of pneumoconiosis complicated with pulmonary infection, isolated and cultured bacterium by conventional methods, then identified bacterium and did susceptibility testing by using BD Phoenix-100 automicroscan microbial identification system, and the drug susceptibility test was carried out by K-B method at the same time. Results: 224 strains pathogens were detected in 744 sputum samples, including 185 strains gram negative bacteria, accounting for 82.6%,25 strains gram positive bacteria which accounted for 11.2%.The major gram negative pathogens were Klebsiella pneumoniae,Escherichia coli, pseudomonas aeruginosa and Acinetobacter baumannii. The major gram positive pathogens were Staphylococcus aureus,Streptococcus pneumonia. The isolates of gram negative bacteria were resistant to Cephalosporins and semisynthetic penicillins, and the isolates of gram positive bacteria were resistant to the second generation and the third generation Cephalosporins. The infection rate is different of the patients in the different stages of pneumoconiosis, the different ages and the different types of pneumoconiosis(P < 0.01).Conclusion: The gram negative bacilli have become the main pathogenic bacteria in pneumoconiosis complicated with pulmonary infection in Zibo area.Higher resistancerates of pathogenic strains prompt us to strengthen monitoring of susceptibility and guide rational drug use. |
