| Objective :Lung function by detecting different GOLD grading the proportion of Th17 cells in peripheral blood in patients with COPD and the level of IL - 17, then compared the cells and cytokines differences between groups and carries on the correlation analysis and lung function, thus preliminarily Th17 cells and IL - 17 on the pathogenesis of COPD, and may, in the process of looking for new targets for clinical intervention .Methods :Choose between January 2016 and February 2016 in the second affiliated hospital of kunming medical university second respiratory medicine ward in clinical remission,82 cases of COPD patients in hospital (of COPD patients hospitalized with acute attack by the treatment of patients after 7 to 10 days) as a case group, aged between 55-85, 72 cases of men and women in 10 cases. Lung function according to the GOLD grade is divided into A, B, C, D four groups, including GOLD Ⅰ level cases in group A, GOLD Ⅱ level for the cases in group B, GOLDⅢ level for the cases in group C, GOLDⅣ level cases D group, A group of 17 cases of patients, 22 cases of group B patients, patients 21 cases of group C, group D 22 cases of patients. Another set up the health control group, control group from June 2016 to December 2016 in our hospital physical examination center examination. A total of 27 cases, aged between 61-61, 19 cases of men, women, 8 cases. According to the smoking group, in which the group for the group E non smoking normal lung function, smoking group F for normal lung function group, group E 14, F group of 13 people. Case group and control group in clinical basic information collection, including age, sex, body mass index, smoking index, the related parameters of lung function (FEV1, FEV1 % Pred),and to detect the proportion of Th17 cells and the level of IL - 17.Results :1. the case group and the proportion of Th17 cells in peripheral blood (2.35% ±1.60%) higher than control group (1.88% ± 1.30%), P = 0.026; The levels of serum IL - 17 cases (89.93 ± 28.50 ng/ml) than the control group (39.30 ± 15.44 ng/ml). P=0.00.2. The control group, the proportion of smoking group F Th17 cells in peripheral blood (1.75% ± 0.96%), than the non-smoking group E (2.00% ± 1.58%) no significant change. P = 0.629; Smoking group F serum levels of IL - 17 (40.00 ±14.65 ng/ml) than the non-smoking group E (38.67 ± 16.75 ng/ml) has no obvious change, P = 0.828.3. in the case group different GOLD lung function classification between groups,3.1 A. B, C, D four groups the proportion of Th17 cells respectively(1.80%±1.48%, 1.93%±1.71%, 2.31%±0.97%, 2.71%±1.56%) . Compared between two groups, group D than in group A than group B, group D. A higher proportion of group C. D group (P values were 0.032, 0.032, 0.036). the other is no significant difference between groups, (AvsB, P = 0.83; AvsC, P = 0.41; BvsC, P =0.414)3.2 A, B, C, D four groups the levels of IL - 17 respectively (81.93 ± 30.15 ng/ml,90.17 ± 30.48 ng/ml,91.49± 25.47 ng/ml,96.85 ± 29.02 ng/ml). Compared between two groups, group D than in group A than group B, group D, A higher proportion of group C, D group (P value were 0.035, 0.039, 0.041), the other is no significant difference between groups (AvsB, P = 0.441; AvsC, P = 0.37; BvsC, P =0.554)4. Pearson correlation analysis, found the proportion of Th17 cells are negatively related with FEV1 (r =- 0.563, P = 0.026), the levels of IL - 17 are negatively related with FEV1 (r =-0.631, P = 0.03).Conclusions:1、Th17 cells and Il-17 play an important role in the development of COPD,it play a proinflammatory role.2、The proportion of Th17 cells in the patients with COPD and the level of Il-17 were raised by a healthy control group, suggesting chronic inflammatory response in COPD remission..3. The proportion of Thl7 cells in the COPD remission, the level of Il-17, and FEV1 were negatively correlated. |
