Objective:1.Analysis of RO resection and R0~+ resection to obtain radical resection satisfaction differences;2.To explore the best surgical approach for surgical treatment of primary liver cancer patients.Methods:Department of Hepatobiliary and Pancreatic Surgery, Second Affiliated Hospital of Kunming Medical University from December 2013 to December 2015 were retrospectively analyzed in 78 patients undergoing surgical treatment and postoperative pathologic examination for primary liver cancer (HCC),Among of them,39 cases of hepatocellular carcinoma, 21 cases of cholangiocarcinoma, 18 cases of mixed liver cancer.According to the different surgical methods, it was divided into RO + resection group, RO resection group and non-anatomical hepatectomy group. The liver function recovery, complication, postoperative tumor markers, mean hospitalization time, recurrence rate and tumor-free survival rate were compared between the three groups.Results:1.Intraoperative blood loss: primary liver cancer surgery, radical resection group(including RO + group and RO group) patients with less intraoperative blood loss than non-anatomical liver resection group of patients.But the operation time was redundant non-anatomic liver resection group. There was no difference between RO + resection group and RO resection group.2.Liver function recovery at 3days and 7days postoperatively: patients undergoing radical resection (including RO + and RO) were significantly faster in patients who underwent non-anatomic hepatectomy.But there was no significant difference between R0~+excision group and RO group.3.The incidence of postoperative complications: patients undergoing radical resection(including RO + and RO) were lower in patients with non-anatomic hepatectomy.The mean hospital stay was shorter in the radical resection group (including the RO + and RO groups).4.Postoperative AFP: Comparison between the three groups, RO + resection group after the fastest decline in AFP.5.1 year recurrence rate and tumor-free survival rate: one year after follow-up, RO +resection group the lowest recurrence rate of 8.3%, tumor-free survival rate of 91.7%.Conclusion:1 .Accurate anatomical RO + radical resection of primary liver cancer surgery has a higher safety and practicality, more suitable for clinical.
