| Objectives:Sepsis-induced Myocardial Depression patients with conventional anti sepsis and for the treatment of myocardial injury based on Xinmailong injection,observe the changes of the relevant indicators,evaluation of clinical efficacy,efficacy and safety evaluation of Xinmailong injection drug use,and provide a theoretical basis for clinical application of xingmailong early treatment of sepsis myocardial depression.Methods:From March 2016 to March 2017,screening in the Kunming Medical University First Affiliated Hospital emergency ICU,after admission by the diagnosis of sepsis in patients with 122 cases,after active fluid resuscitation,in line with sepsis myocardium Suppression of 67 cases,according to the relevant inclusion,elimination,shedding criteria,36 patients were included in the clinical observatio study.According to the Department of the order and were randomly divided into experimental group(Xinmailong group)(n=17)and control group(n=19),15 cases were female and 21 were male,age between 23-76.Two groups of patients were given conventional anti infection,fluid resuscitation,improve cardiac function and other treatment,were deep vein puncture into the PICCO catheter connected to the PICCO monitor for onitoring,On the basis of this,the experimental group was treated with Xinmailong injection of 5mg/kg + 0.9%sodium chloride injection 200ml intravenous infusion,2 times a day for 5 days;the control group by adding 0.9%sodium chloride injection 200ml intravenous Infusion,2 times a day,continuous use for 5 days.The levels of troponin I and B brain natriuretic peptide were measured at before the treatment,1 day,3 days and 5 days after the start of treatment,and the peripheral indexes were monitored by Doppler chocardiography.Before and after treatment for fifth days,peripheral venous blood was taken to detect the function of liver and kidney,and the heart rate and mean arterial pressure were recorded and analyzed.The experimental data were statistically analyzed by SPSS 19.0 statistical software package.Results:1、Two independent samples t test:There was no significant difference in age,height and APECHEII score between the experimental group and the control group(P>0.05).There was no significant difference in gender,disease type and blood evolution between the two groups(P>0.05).The baseline was comparable.2、Repeated data variance analysis:BNP,troponin I between the main body,the main effect of the test differences were statistically significant(P<0.05);LVEF,E/A peak,EDT,TAPSE test group and the control group,the main body of the test results were statistically significant(P<0.05),There was no significant difference in the subjects between left ventricular end-diastolic meridian and left ventricular endometrium(P>0.05).SVI,SVRI,CFI test group and the control group,the main body of the test results were statistically significant(P<0.05),About the GEDI,There was no significant difference between the experimental group and the control group(P>0.05).There was significant difference between the two groups(P<0.05).3、Two independent samples t test:There was no significant difference in liver function,heart rate and mean arterial pressure between the two groups before and after treatment(P>0.05).4、Two independent samples t test:There were significant differences in the duration of mechanical ventilation,ICU days and dobutamine levels between the two groups(P<0.05),There was no significant difference in all-cause mortality between hospitalizations(P>0.05).5、According to the direct record,the experimental group appeared 1 cases of tachycardia,no other adverse reactions.Conclusions:1、Xinmailong injection can significantly reduce the sepsis-induced myocardial depression patients with BNP,cTnl value,can improve cardiac systolic and diastolic function.Have a certain degree of myocardial protection.2、Xinmailong injection had no effect on liver and kidney function and life sign,no obvious adverse reaction.Can be used as a useful supplement for the treatment of sepsis-induced myocardial depression. |
