BACKGROUND:"Competence" is the standard of distinction between excellent and ordinary people from the individual's deep-level characteristics,which can be personality,traits,motives,attitudes or values and cognition or behavior[1].Training competency is an important way to become an excellent doctor,so medical education reform direction should be adjusted accordingly[2].In recent years,more and more researches have focused on the topic of "competence-based" education.The relevant evaluation model of competency researched by foreign medical education experts is more comprehensive,and has been widely used in practical education[3-4]However,because of different national conditions,it's difficult to be applied in our country.Domestic research on the competence of undergraduate students in clinical medicine started late[5-6].Although some significant results have been achieved,there still are many problems.At present,there is no specific research on the competence evaluation of undergraduates of clinical medicine in Guangdong province.If the clinical medicine undergraduate education reform and innovation in Guangdong Province want to go in the forefront,it is necessary to process relevant researches and explorations.Based on what's mentioned above,the aim of this study is to construct the evaluation index system regarding the competence of undergraduates majored in clinical medicine in Guangdong province.This system is used to provide evaluation tools for the scientific and rational evaluation of clinical undergraduate students,provide a characteristic model for training excellent clinical undergraduate students,and lay the foundation for "competence-based" education.OBJECTIVE:To establish a scientific and reasonable competence evaluation system for undergraduates of clinical medicine in Guangdong Province,and to measure the effectiveness of the index system.METHODS:Literature research,brainstorming,behavioral event interview,questionnaire survey and Delphi.1.Constructing the draft of competence evaluation index system for undergraduates of clinical medicine in Guangdong ProvinceIn this study,the literature research method is widely used to review the literature,reports and books about the competency model,to be familiar with the relevant theoretical knowledge and collate relevant contents,to screen out the characteristic entries of the excellent clinical undergraduates,and to construct the theoretical framework of competence evaluation index system for undergraduates of clinical medicine in Guangdong Province.On this basis,select 60 clinical undergraduate interns in the A hospital,a large hospital in Guangzhou City,and then collect characteristic terms by the method of brainstorming.In order to ensure the comprehensive of characteristic terms,select 10 outstanding clinical first-line doctors in the A hospital,organize behavioral interviews and further collect indicators.According to the results of literature research,brainstorming and behavioral interviews,the index pool is constructed.Finally,select 20 experts(including clinical medicine teachers in the university,hospital staff,hospital clinicians)in the medical universities and large-scale AAA hospitals in Guangzhou,and conduct a questionnaire survey to screen out the index pool and build the first draft of the index system.2.Establishing the competence evaluation index system for undergraduates of clinical medicine in Guangdong ProvinceSelect and consult 32 experts with senior titles and work experience for more than 10 years in some medical universities and large-scale AAA hospitals in Guangdong Province by the method of Delphi to establish the index system ultimately.3.Measuring the competence evaluation index system for undergraduates of clinical medicine in Guangdong ProvinceThe questionnaire was conducted in the form of self-evaluation,and the eight-year students of clinical medicine(regarded as the undergraduates in the internship period)and the general class of undergraduates were selected as the measured subjects,which are divided into excellent group and ordinary group.After comparing the results of self-assessment between two groups,we can determine whether this index system is effective.RESULT:1.Constructing the competence evaluation index system for undergraduates of clinical medicine in Guangdong ProvinceIn this study,we constructed the the competence evaluation index system for undergraduates of clinical medicine in Guangdong Province,combining 4 first-level indicators of "knowledge and skills","values and service characteristics","self-concept and consciousness","personality quality and motivation" with 34 second-level indicators of analysis and thinking ability,learning ability,basic knowledge of medical science,business ability,problem-solving ability,information collection and processing,English level,legal knowledge,desire for achievement,high demand for quality,the communicative competence with patients and the awareness of service by means of literature research,brainstorming,behavioral event interview,questionnaire survey and Delphi method.2.The preliminary validation of the competence evaluation index system for undergraduates of clinical medicine in Guangdong ProvinceBased on the evaluation index system,the self-evaluation scale of competency for undergraduates of clinical medicine in Guangdong Province was established.By grouping and comparing with each other,200 students of eight years of clinical medicine were selected as excellent group and 200 general clinical medicine subjects born as an ordinary group.There were 400 questionnaires issued to test self-evaluation of subjects and make t test(P<0.05).The results showed that the competency evaluation index system was different between the excellent group and the ordinary group,and the validity of the index system was identified.It is proved that the index system can be used to evaluate the competency of undergraduates of clinical medicine in Guangdong province.CONCLUSION:In this study,the competence evaluation index system for undergraduates of clinical medicine in Guangdong Province was established by using various methods,and the validity of the evaluation system was proved.The results showed that the index system could be used to evaluate the competence of undergraduates in Guangdong province.Filled the gaps in this field in Guangdong Province.The construction of the index system can provide a evaluation tool for scientific and reasonable evaluation of the competence of undergraduates in clinical medicine,and provide a characteristic model for cultivating "excellent" clinical undergraduates,laying the foundation for the "competence-based" education.However,due to the limitations of research time and conditions,this study also exists some problems,such as in the expert consultation,the authority of experts under maximization and lacking of comprehensive measurements to test the effectiveness of the index system.These aspects will be improved in the further study.
