Objective:To explore the application the cross-sectional area, conjugate diameter of cervical spinal cord and DTI with 3.T MRI scanner in cervical spondylotic mylopathy.Materials and methods:31 patients with cervical spondylotic mylopathy were selected as subjects,at the same time,10 normal control groups were selected.According to clinical severity, there were 14 mild,17 moderate, all patients can fond the staitness of spinal dural sac and extrusion of spinal cord.Conventional T1 WI and T2 WI imaging and DTI of cervical spinal cord were performed in all the objects by using Siemens Magnetion Trio A Tim 3.0T MR.A head and neck joint phased array surface coil was accepted. All the raw date were transmitted into Siemens workstation, the disc corresponds to the level of the spine cord were selected to analyze the cross-sectional area, conjugate diameter of cervical spinal cord and the values of ADC and FA were measured at most seriously compressed level of the cervical spinal cord.Regions of interest(ROI)were manually set on FA and ADC map, which should include the entire cervical spinal cord. All the data of objects were measured by two experienced radiologists. All the objects were graded by JOA by two experienced orthopedist. The data were analyzed statistically by using the SPSS 19.0 software package.]Normal control group, mild group and moderate group of data were performed an analysis of variance and Least Significant Difference(LSD)tests. The correlation between conjugate diameter, compression area,FA value and ADC value and JOA score was examined by multiple linear regression analysis. Results:Imaging of all subjects could clearly display myeloid tissue and there was no evident twist and deform of imaging. The FA value,ADC value, spinal cord cross-sectional area and anteroposterior diameter of the dural sac were significantlydifferent between the normal control group and the mild group and the moderate group CSM.Compared with the mild group and the moderate group,Diameter and control group and mild group spinal cord compression area didn’t got statistically significant differences(P >0.05).The other groups were statistically significant.FA value and spinal cord cross-sectional area were correlated with JOA score(r=0.560,P =0.000;r=0.293,P =0.045),and there was no significant correlation between ADC value and conjugate diameter and JOA score. Conclusion:Cervical spinal cord diffusion tensor imaging can be used to detect spinal cord cervical spondylotic myelopathy and contribute to the diagnosis of cervical spondylotic myelopathy.FA values were associated with the highest degree of JOA score, suggesting that FA values can be used to assess preoperative clinical dysfunction in CSM patients, suggesting that FA values can be used as a reliable indicator of assessing the degree of functional impairment in CSM patients.
