Background And Objective:Follicular fluid is a microenvironment that oocyte relies on for survival,it contains nutrients in general,steroid hormone and various bioactive substances,which have important effects on follicular development and oocyte maturation,a premise of for a good pregnancy outcome.Follicle-stimulating hormone(FSH)and luteal hormone(LH)are secreted by pituitary,have amsynergistic effect on activating granulosa cell proliferation and hormone secretion,which are necessary for the development and mature of follicular.At present,most of the research is to observe the relationship between peripheral blood FSH、LH levels and ovarian reserve and to predict the outcome of ovulation induction.whatever the level of FSH and LH in the target organs of follicle maturation period and its relationship with assisted reproductive outcome remains to be studied.This paper mainly studies the relationship between the level of FSH and LH in follicular fluid on oocyte retrieval day and the outcome of IVF/ICSI-ET.Methods:The follicular fluid were obtained on oocyte retrieval day from 111 patients undergoing IVF/ICSI-ET therapy in our center.Enzyme linked immunosorbent assay(ELISA)was used to determine the levels of FSH and LH.1.The FSH and LH levels in follicular fluid were analyzed in relation to the basic situation of the patients,the level of basal hormones,serum E2 level in HCG injection day,drug use and number of oocyte.2.The transplanted patients were divided into pregnant group and non pregnant group according to whether they got the pregnancy.basic situation of the patients,the level of basal hormones,serum E2 level in HCG injection day,drug use,number of oocyte were compared between the two groups.3.All samples were divided into three groups according to the level of FSH in follicular fluid: group A(FSH﹤3.0IU/L),group B(FSH3.0~7.5 IU/L),group C(FSH﹥ 7.5 IU/L).Analyze whether there differences in basic situation of the patients,number of oocyte,fertilization rate,high quality embryo rate,multiple PN fertilization rate,implantation rate and clinical pregnancy rate.Results:1.The level of FSH and LH in follicular fluid was positively correlated with the amount of Gn per effective follicle,and negatively correlated with the number of oocytes(P < 0.01)2.There was significant difference in fertilization rate,high quality embryo rate and level of FSH in follicular fluid between pregnant group and non pregnant group(P﹤0.01),while other indicators have no different(P﹥0.05).3.The high quality embryos rate and clinical pregnancy rate in group B was statistically higher than in group A and C(P﹤0.05).The fertilization rate in group B was statistically higher than in group C(P﹤0.05).Conclusion:The level of FSH in follicular fluid was correlated with IVF/ICSI-ET outcome,and when concentration was 3.0~7.5 IU/L the outcome will reach the best.In this experiment,the LH level in follicular fluid of patients was lower,it was hardly evaluated the correlated with IVF/ICSI-ET outcome. |
