| Background:Gastrointestinal eminent lesion is a disease that presents as gastrointestinal submucosal lesions or thickness of the gastrointestinal tract.Endoscopic ultrasonography is a very important examination method in the diagnosis of gastrointestinal tract and surrounding organ diseases,which also provide a valuable image information of the gastrointestinal eminent lesion.Mini-probe endoscopic ultrasonography(mEUS)is a type of endoscopic ultrasonography.The advantages of mEUS in gastrointestinal eminent lesion diagnosis and treatment is that: easy to operate,examine and treat the disease at the same time,lessen the patient’s pain by reduce the frequencies of the examination.Purpose:This study was designed to investigate the accuracy of mEUS in the diagnosis of the gastrointestinal eminent lesions,and to summarize and discuss the characteristics of mEUS image of gastrointestinal eminent lesions,so as to comprehensively evaluate the ability of m EUS in the diagnosis of gastrointestinal eminent lesions.Method:Retrospective analysis of 302 cases of patients with gastrointestinal eminent lesions that were subjected to mEUS treatment.All diagnosis results were verified by either endoscopic pathology,surgical pathology or follow-up diagnosis.Compare the diagnostic accuracy of the mEUS’s and traditional endoscpic findings.The data were analyzed by SPSS13.0,and the percentage was calculated by percentage.The odds ratio was compared with χ2 test.Results:A total of 302 patients underwent mEUS,among that,274 cases of patients were examined in upper gastrointestinal tract,28 cases in colorectal tract.In the cases of upper gastrointestinal tract,there are 97 cases of esophagus,152 cases of stomach and 25 cases of duodenum.The accuracy of mEUS in the diagnosis of the eminent lesions of esophagus was 97.93%(95/97),and the accuracy of gastroscope was 68.04(66/97).In the 97 cases of esophagus eminent lesions,there are 42 cases finally diagnosis as leiomyoma or stromal tumor,7 cases liopmas,10 cases cyst,9 cases isolated esophageal varices and 13 cases external pressure lesion.The accuracy of mEUS in the diagnosis of gastric eminence lesions was 94.07%(143/152),and the accuracy of endoscopy was 50.65%(77/152).There were 15 cases of gastric mucosal thickness among 152 cases of gastric eminence lesions,the other were consists of 1 case of gastric linitis plastic,5 cases of lymphoma,7 cases of gastric varices,1 case of lymphocytic gastritis and 1 case of racemose hemangioma.135 patients were of gastric submucosal lesions,included 36 cases of gastric external pressure lesions,1 case of neuroendocrine tumor,13 cases of ectopic pancreas,75 cases of stromal tumor / leiomyoma,4 cases of hemangiomas,4 case of cyst,1 case of fibroma,5 cases of angeioma,1 case of gastric hepatoid adenocarcinoma.The accuracy of mEUS in the diagnosis of duodenal eminent lesion was 88%(22/25),and the accuracy of endoscopic was 68%(17/25).The 25 cases of Duodenal eminent lesions included 1 cases of duodenal lipoma,4 case of duodenal stromal tumors,8 cases of duodenal external pressure lesions,6 cases of duodenal cyst,1 case of duodenal neuroendocrine tumor,1 cases of duodenal Brunner’s gland tumors,4 cases of duodenal ectopic pancreas.The accuracy of mEUS in the diagnosis of colorectal eminent lesions was 92.85%(26/28),and the accuracy of endoscopy was 57.14%(16/28).The 28 cases of colorectal eminent lesions were consist of 3 cases of colorectal neuroendocrine tumors,5 cases of colorectal lipoma,4 cases of colorectal stromal tumor / leiomyoma,1 case of colorectal cendometriosis,4 cases of colorectal cyst,3 cases of colorectal pneumatosis coli,6 cases of external pressure lesion,2 cases of colorectal lymphoma.Conclusion:Mini-probe endoscopic ultrasonography can clearly show the structure of the gastrointestinal wall,reflect the origin and the invision depth of the lesions,so it can make an accurate diagnosis to the majority of gastrointestinal eminent lesions. |
