| Objective: To observe the changes of Th22 and as well as cytokines interleukin-22(IL-22)levels in peripheral blood of patients with coronary heart disease(CHD),and to analyse the relationship between Th22 and IL-22 in patients with CHD and the correlation with coronary artery disease.To investigate the mechanism of Th22 and IL-22 in the patients with CHD.To provide a new index for the diagnosis of CHD,and to provide a faster and more effective method to evaluate the degree of coronary artery disease.Methods: 60 patients with coronary heart disease in Jingzhou Central Hospital from November 2015 to September 2016 were enrolled,and 20 patients with normal coronary angiography(During the same period)were enrolled as healthy control group.There were 49 males and 31 females,aged from 54 to 81 years,with an average age of 65.00±8.12 years old.Among them,patients with CHD were divided into three groups according to the relevant diagnostic guidelines,a total of 60 CHD patients was divided into stable angina pectoris(SAP)group(n=20),unstable angina pectoris(UAP)group(n = 20),and acute myocardial infarction(AMI)group(n=20).20 healthy controls(HC)were normal coronary angiography patients,Physical examination of 20 healthy control group: such as blood routine,blood lipid,blood sugar,liver and kidney function were normal,exercise load and ECG were negative.There were no significant differences in age,sex,smoking,drinking,hypertension,body mass index(BMI),diabetes mellitus,dyslipidemia and so on(P>0.05)among 60 patients,which was comparable.And all objects to exclude congenital heart disease,pulmonary heart disease,rheumatic heart disease and other severe valvular heart disease,cardiomyopathy,associated with malignant tumor,heart failure,liver and kidney dysfunction,trauma,infection,peripheral vascular disease,autoimmune disease,and 3 months for cardiac surgery or interventional surgery.Four groups of patients were extracted venous blood.Blood sampling time: AMI group was within 24 h after onset,group UAP,group SAP and group HC were extracted in the early morning fasting state.All specimens were centrifuged in 30 minwith 3000r/min for 10 min.The sealing seal after the separation of serum,and to store in a-80 Celsius low temperature refrigerator for testing.The percentage of peripheral blood Th22 cells in CD4+T cells was detected by flow cytometry and the concentration of plasma IL-22 were measured by Enzyme linked immunosorbent assay(ELISA)of each group.Coronary angiography was performed in patients with CHD,according to the results of coronary angiography and then using the Gensini scoring system to evaluate the severity of coronary artery disease.Results: The Th22 cell ratio in CD4+T cell and the concentration of plasma IL-22 in group AMI[(2.02±0.84)%,(39.12±5.41)pg/ml]were significantly higher than that of SAP group[(0.74±0.27)%,(28.14±3.78)pg/ml],UAP group [(1.20±0.34)%,(33.50±3.47)pg/ml]and HC group [(0.48±0.19)%,(25.38±3.13)pg/ml],the difference were statistically significant(P<0.05).There were difference between UAP group and SAP group,UAP group and HC group(P<0.05);there was difference between SAP group and HC group(P<0.05).The proportion of Th22 cells in AMI group,UAP group and SAP group were significantly positively correlated with IL-22 concentration,γ were respectively 0.764,0.877 and 0.968,p<0.001.Th22 and IL-22 were significantly positively correlated with Gensini score,person analysis,γ were respectively 0.889 and 0.962,p<0.001.Conclusion:(1)Th22 and as well as cytokines interleukin-22 were involved in the development of coronary atherosclerotic heart disease,the levels of Th22 and IL-22 reflect the degree of atherosclerotic plaque in acute inflammation.(2)There was a positive correlation between Th22 and IL-22 in all groups,suggesting that the IL-22 involved in inflammatory response is mainly produced by Th22 in coronary heart disease.(3)Th22 and IL-22 were positively correlated with Gensini score,which could indirectly reflect the degree of coronary artery stenosis.To sum up: the detection of Th22 and IL-22 could be used as an auxiliary method for the diagnosis of coronary heart disease,and the level of Th22 and IL-22 could be used as a noninvasive and safe index for judging the degree of coronary stenosis. |
