The Clinicalapplication Of Pneumovesicum En Bloc Resection For T₁-T₂ Bladder Tumor

Objective To compare the efficacy and safety of pneumovesicum En bloc resection of bladder tumors（PEBRBT）with partial cystectomy（PC）in treatment of T₁—T₂ bladder cancer.Methods We reviewed 108 patients with primary T₁—T₂bladder tumor in our hospital,who were divided into PEBRBT groups（n=43）and PC groups（n=65）from March 2014 to March 2016.Instillation therapy with epirubicin（EPI）were taken postoperative T₁ staging tumor total 12 months and chemotherapy with gemcitabine/cisplatin were introduced for T₂ tumor for 4⁶ courses of treatment.Cystoscope were taken per 3month The operation time,complication,postoperative gross hematuria and hospital stay were compared between the two groups.The survival rate without recurrence of two groups were statistical analyzed by using Kaplan-Meier methodResult Two groups both accomplished.The operative time of pneummovesicum En bloc group was shorter than PC group [（29.40±2.4）min VS（38.8±4.50）min,P<0.05].The postoperative gross hematuria and hospital stay were no significant difference in two groups Obturator nerve reflex was not in both pneummovesicum En bloc group and PC group..Both groups were followed up for 12 to 24 months with a medium of 18 months,which pneummovesicum En bloc group showed lower rate of recurrence than PC group[ pneummovesicum En bloc VS PC 14.0%（6/43）VS 32.3%（23/65）,].Kaplan-Meier analysis revealed that pneummovesicum En bloc group raised the survival rate without recurrence（Log Rank X2=5.219,P=0.022,）.Conclusion 1.CO₂ pneumovesicum En bloc resection of bladder tumors（PEBRBT）is safe and effective in treatment ofT₁—T₂bladder cancer,which could reduce complication,and lower recurrence of tumor.Compare to PC,PEBRBT has the advantage of minimally invasive,fast recover,shorter hospital stay.2.CO₂ pneumovesicum En bloc resection of bladder tumors（PEBRBT）and partialcystectomy（PC）have similar indication in treatment ofT₁ — T₂ bladder cancer.By mastering the indication of treatment,the approach could be improved.
