Objective: To explore the possible etiology of acute renal injury caused by upper urinary tract obstruction and to analyze the correlative risk factors influencing the recovery of renal function,providing a basis for better prevention of acute renal injury and improving prognosis of patients.Methods:This research retrospectively gatherted the clinical data of p atients with obstructive acute kidney injury caused byupperurinary tractobst ruction in urology from the First Affiliated Hospital of Guangxi Medical University from January 2012 to December 2016.According to strict inclus ion and exclusion criteria,perfectlyscreening out the 118 cases of AKI pat ients,Retrospectively review the clinical data of patients with AKI,specific indicators are as follows: the clinical data of patients basic condition,rel ated examination,treatment methods and prognosis,etc.1months after oper ation as cutoff point..According to the recovery of renal function in patien ts with AKI,The prognosis of AKI patients was divided into renal healing Group,partial healing group and three groups of poorly healed group.St atistical analysis of the independent risk factors of prognosis of patients w ith AKI by SPSS16.0 statistical software.The main statistical methods of t test,chi-square test,Univariate and multivariate Logistic regression analysis.P<0.05 is as statistical significance.Results:Observation on 118 cases of upper urinary tract obstruction in the etiology of acute renal injury mostly induced byupper urinary tract calculi,accounting for 88.14%,followed by metastasis or invasion,7.63%,other causes of acute kidney injury in 4.24%,such as inflammatory stenosis of ureter,renal graft distal ureteral stenosis and hydrocephalus.Etiology of patients with acute kidney injury caused by upper urinary stones,Ureteral calculi of solitary kidney in 52.88%(55/104),bilateral ureteral stone in34.62%(36/104);side of ureteral calculi,stones on the other side of 4.8%(5/104);7.69%8/104)side of ureteral calculi.Independent sample t test analysis of renal function recovery and patient age,serum potassium concentration(K +),hemoglobin(Hb)and other general clinical data was not statistically significant,Univariate Chi-square test analysis indicated that postoperative polyuria occurs,AKI stage,RRT and kidney prognosis were statistically significant,but only after multivariate logistic regression showed AKI stage,RRT kidney prognosis were statistically significant.Conclusions:Intheetiologyofacutekidney injury caused byupperurinary tractobstruction,urinary tractstones is the most common actor,thentumors metastasisorinvasion.AKI clinical stage for patients with acute kidney injury is poor prognosis risk factors,RRT isa protective factor of acute kidney injury.
