Clinical Analysis Of Standard Breast Conservation Therapy And Oncoplastic Breast Conserving Therapy For Early Breast Cancer

Background: Many studies have confirmed that breast conserving patients was significantly higher than that of patients who breast resection in the quality of life and satisfaction with treatment.The NSABP B-06 test showed early breast cancer with breast conserving therapy(BCT)can get the same survival of radical operation.Oncoplastic breast conserving therapy(OBCT)is on the basis of standard breast conservation therapy(SBCT)by using plastic surgery technique.A radical cure of the tumor at the same time to maximize the retention of the shape of the breast.The effect of postoperative treatment has been much attention.Objective: To compare the therapeutic effect、recurrence and metastasis rate of early breast cancer between SBCT with OBCT.Methods: Collecting 190 patients with breast cancer who underwent BCT from January 2010 to October 2015 who come from Breast Cancer HospitalAffiliated to Guangxi Medical University,including 121 patients taking SBCT,69 cases taking OBCT.The clinical therapeutic effect and the short term recurrence and metastasis rate of two groups were analyzed.Results:(1)Among the 190 patients undergoing BCT,69 patients underwent OBCT(36.3%),of which: "Ω" tumor resection of breast resection in13 cases,inverted "T" tumor resection of breast resection in 9 cases,4 cases of pedicle flap breast plasty,22 cases of balloon incisional tumor resection of breast resection,13 cases of resection of breast tumor by double loop method,and 8 cases of adjacent flap transfer repair.(2)Methods 116 cases of SBCT group were counted and 67 cases of OBCT group statistics with resected specimens were the average volume of tumor was 56.98±9.58 ml vs.95.35±11.23 ml.The minimal tumor resection margins in the postoperative pathological reports were 9.59±1.70 mm vs.12.03±2.76 mm,The maximum margins of the tumors were 15.25±2.73 mm vs.24.94±9.73 mm.Their P values are less than0.05,the difference was statistically significant.(3)Some complications after surgery,the SBCT group combined the OBCT group seroma occurred in 48.8%vs.14.5%,the incidence of hematoma was 14.9%,vs.4.3%,the incidence of poor wound healing was 11.6%,vs.3%.The incidence of serum swelling,hematoma and wound healing in the breast conserving surgery group was lower than that in the conventional breast conserving surgery group,and the difference was statistically significant(P < 0.05).(4)In the comparison of breast satisfaction,the satisfaction rate of the SBCT group was 80.2%,the satisfaction rate of the OBCTgroup was 92.8%,the difference was statistically significant(P < 0.05).In the objective evaluation of cosmetic results after mastectomy,the excellent and good rate of the SBCT group was 67%,and the the OBCT group was 87%.The excellent and good rate of the OBCT group was higher than that of the SBCT group,the difference was statistically significant(P < 0.05).(5)After 5 years the local recurrence rate of the SBCT group was 4.1%,the rate of metastasis was4.1%.After 5 years the local recurrence rate of the OBCT group was2.5%,the rate of metastasis was 5%.The local recurrence rate and the distant metastasis rate of the OBCT group were similar to the SBCT group after 5 years,and the difference was not statistically significant(P > 0.05).Conclusion: Compared with SBCT,the OBCT for early breast cancer can achieve wider surgical margins,and greater tissue resection.The OBCT part of short-term complication of seroma,hematoma and poor wound healing rate is lower than SBCT,the differences betwween seroma,hematoma and poor wound healing were statistically significant.There was no significant difference in forward complications.The local recurrence rate and distant metastasis rate were not significantly different between the 5 years after the operation,and satisfactory postoperative cosmetic results can be obtained,It is a safe,effective and worthy of breast conserving operation.
