Objective: To investigate the mechanism,risk factors and prognosis of complications after decompressive craniectomy for severe traumatic brain injury,and provide evidence for the prevention and treatment of complications after decompressive craniectomy.Method: A retrospective analysis from June 2014 to June 2016 in the First Affiliated Hospital of Guangxi Medical University were admitted to the Department of Neurosurgery,95 cases underwent decompressive craniectomy in patients with severe craniocerebral trauma.Collect and analyze their clinical data: gender,age,etiology,disease history,admission GCS score,signs,imaging data and prognosis.Result:In 95 patients,77 male patients(81.05%),18 cases of female patients(18.94%),age range 6-77 years old,the average age is 45.42 years old.The most common disease for 56 cases of traffic accidents(58.95%).30(31.58%)cases after decompressive craniectomy in patients with complications,one of the most common complications is 18 cases of subdural effusion(18.95%).33 patients died(34.74%),delayed intracranial hemorrhage mortality was highest.Univariate analysis: age,admission GCS score,hematoma location,change of pupil before surgery,and midline complications are closely related;Logistics regression analysis showed that age,pupil change,operation the hematoma position,midline shift greater than 10 mm are independent risk factors of severe craniocerebral injury after decompressive surgery complications.Conclusion: Occurrence of severe traumatic brain injury after decompressive craniectomy common complications,delayed intracranial hemorrhage seriously affect the prognosis of patients,postoperative close observation,early detection and early treatment.To the patients who were older,more serious craniocerebral trauma,postoperative complication rate is high.
