| Objective:Under normal circumstances,general anesthesia after stopping the administration,the patient can be completely awake within 30 minutes in clinical work.But still a small number of patients have not yet recovered after stopping the drug 90 minutes,this situation is considered delayed postoperative recovery.This paper collects the analysis of patients who had postoperative recovery after general anesthesia liver resection in our hospital,to explore the different use of drugs during surgery,different basic diseases,the use of EEG bispectral index to monitor and other related factors on general anesthesia,and to provide reference to patients.Methods: In this paper,the data selected from 2014 to 2016 Guangxi Medical University First Affiliated Hospital of Eastern Hospital operating room under general anesthesia liver tumor resection of patients with electronic medical records,anesthesia record and recovery record.The age of patients is between 18 and 65 years.Exclusion condition: incomplete data(eg missing partial hemodynamic parameters,not have the data type required for this monitoring,incomplete medical records,etc.).Logistic regression was used to analyze the data of clinical cases in order to explore the related factors of postoperative recovery of liver tumor were analyzed.Results: Influencing variables of delayed postoperative recovery such as age,sex,weight,height,liver function classification,anemia,hypertension,diabetes,history of drinking,history of smoking,history of lung disease,general anesthesia,anesthesia,ASA grading,surgical grade,whether combined inhalation anesthesia,whether combined epidural anesthesia,intraoperative propofol,remifentanil dosage and fentanyl dosage,total infusion volume,intraoperative blood transfusion,whether using BIS monitoring Anesthesia depth,postoperative immediate temperature,a total of 24 items.After the univariate analysis of the above variables,the multivariate log regression analysis was performed by screening(P<0.05).The results showed that there were 8 factors affecting the recovery of the patients after hepatectomy,The difference was statistically significant(P<0.05).(OR = 2.408),age(OR = 1.909,P = 0.028),and the time of anesthesia(OR = 2.488,P = 0.034),age(OR = 1.409,P = 0.028)(OR = 1.378,P = 0.017),anemia(OR = 1.251,P = 0.002),total intraoperative infusion(OR = 1.658,P = 0.03),smoking(OR = 1.93,P = OR = 1.177,P<0.001)and BIS monitoring(OR = 0.298,P<0.001)Conclusion : 1.With the liver tumor resection in patients with age,anesthesia duration,intraoperative infusion volume and intraoperative propofol dosage increased,the risk of postoperative recovery delay increased.2.postoperative hypothermia,alcohol,smoking,anemia and the use of BIS monitoring during surgery are the independent risk factors of patients with postoperative recovery delay. |
