Objective: To observe the effect of low-frequency ultrasound irradiation treatment on wound healing in diabetic rats and to explore its possible mechanism,to provide a theoretical basis for the new treatment for diabetic refractory wound.Methods: 45 healthy male Wistar rats were divided into 3 groups randomly,namely ultrasonic therapy group 15,routine treatment group 15,blank control group 15,the rats received intraperitoneal injection of streptozotocin(STZ)induced diabetic rat model.Diabetic rats were successfully modeled and stabilized for 3 weeks to establish the rat model of full-thickness skin defects.The ultrasonic treatment group was treated with physiological saline change,low-frequency ultrasound irradiation 15 minutes,once a day,the routine treatment group was given physiological saline change once a day,the blank control group without any treatment.To observe the wound situation of all rats after Wound model day1,day 7,day 14 and day 21,and calculate the group wound healing rate.Taking the wound tissue to observe the situation of the inflammatory cell,fibroblasts,collagen and angiogensis in HE staining.Real-Time PCR was to detect the expression levels of mRNA of TGF-β1,VEGF,TNF-α and IL-6.The data were analyzed by SPSS 23.0 software.Results:1.The general condition of the rats: normal rats,body fat,hair gloss,frequent nocturnal;the diabetic rats,listlessness,less activity at night,showing diabetic typical symptoms such as weight loss,polydipsia,polyuria,polydipsia of the typical " a little more than three " symptom.2.Wound healing and wound healing rate: wound healing rate and healing situation of ultrasound treatment group was better than that of the other two groups,wound inflammation gradually subsided,granulation tissue grew faster,the wound gradually narrowed;routine treatment group wound inflammation subsided slowly,granulation tissue grew slowly,wound healing slowly;blank control group wound infection,visible necrotic tissue,wound healing was very slow.The wound healing rate of ultrasonic treatment group was higher than that of routine treatment group and blank control group on day 7,14 and day 21,the difference was statistically significant(p<0.05).3.HE staining showed that: The angiogenesis,fibroblast proliferation and collagen fiber synthesis of ultrasound treatment group were better than that of the other two groups,and the inflammatory cells decreased faster than the other two groups.The inflammation subsided faster than the other two groups.4.Real-time PCR showed that:TGF-β1: The wound tissue relative expression of mRNA of TGF-β1 in each group was low on the first day,it was not statistically significant;the relative expression was elevated on day 7 in each group,ultrasonic therapy group was higher than the other two groups,but it was not statistically significant;the relative expression was further increased on day 14 in each group,ultrasonic treatment group was higher than the other two groups,and it was statistically significant(p<0.05);the relative expression was decreased on day 21 in each group,but ultrasonic treatment group was still higher than the other two groups,and it was statistically significant(p<0.05).VEGF: The wound tissue relative expression of mRNA of VEGF in each group was low on the first day,it was not statistically significant;the relative expression was elevated on day 7 in each group,ultrasonic treatment group was higher than the other two groups,but it was not statistically significant;the relative expression was further increased on day 14 in each group,ultrasonic treatment group was higher than the other two groups,and it was statistically significant(p<0.05);the relative expression was decreased on day 21 in each group,but ultrasonic treatment group was still higher than the other two groups,and it was statistically significant(p<0.05),routine treatment group was higher than blank control group,and it was statistically significant(p<0.05).TNF-α: Wound tissue relative expression of mRNA of TNF-α in each group was low on the first day,it was not statistically significant;Each group was higher than before on day 7,ultrasonic treatment group was lower than the other two groups,it was not statistically significant compared with routine treatment group(p>0.05),it was statistically significant compared with blank control(p<0.05);ultrasonic treatment group and routine treatment group were decreased on day 14,ultrasonic treatment group was lower than routine treatment group,and it was statistically significant(p<0.05),while blank control group was higher than before;ultrasonic treatment group and routine treatment group were further reduced on day 21,ultrasonic treatment group was lower than routine treatment group,and it was statistically significant(p<0.05),while control group was increased again.IL-6: The wound tissue relative expression of mRNA of IL-6 in each group was low on the first day,it was not statistically significant;Each group was higher than before on day 7,ultrasonic treatment group was lower than the other two groups,it was not statistically significant compared with routine treatment group(p>0.05),it was statistically significant compared with blank control group(p<0.05);ultrasonic treatment group and routine treatment group were decreased on day 14,ultrasonic treatment group was lower than routine treatment group,and it was statistically significant(p<0.05),while blank control group was higher than before;ultrasonic treatment group and routine treatment group were further reduced on day 21,ultrasonic treatment group was lower than routine treatment group,and it was statistically significant(p<0.05),while control group was increased again.Conclusions:1.Low frequency ultrasound irradiation treatment can promote angiogenesis,fibroblast proliferation and collagen synthesis,promote granulation tissue growth and promote wound healing in diabetic rats.2.Low frequency ultrasound irradiation treatment can increase the expression of mRNA of vascular endothelial growth factor(VEGF)and transforming growth factor-β1(TGF-β1)in diabetic rats’ ulcer wound.3.Low frequency ultrasound irradiation treatment can decrease the expression of mRNA of tumor necrosis factor alpha(TNF-α)and interleukin-6(IL-6)in diabetic rats’ ulcer wound,promote inflammation subsided.4.Low frequency ultrasound irradiation treatment is safe,noninvasive and effective.
