Objective: To study the change rule of hemodialysis patients in blood pressure,blood dialysis related parameters influence on blood pressure in patients on dialysis,design the study of the clinical analysis.This study adopts the method of retrospective study,30 cases of blood pressure changes in maintenance hemodialysis patients dialysis and ultrafiltration volume,standardized ultrafiltration volume,time,age,age of dialysis patients,hemoglobin content,primary disease,deficiency under different variables,such as blood pressure change trend has carried on the induction summary analysis,to explore some blood pressure change law of hemodialysis patients.Methods:Adopted retrospective study method,this study selected the law of 30 cases of hemodialysis patients in recent 36 weeks of time a total of 3049 cases of times dialysis condition change,the data records,medical records,grouping contrast.According to the patient in the 36 weeks of dialysis,the blood pressure changes in divided into low blood pressure,high blood pressure and blood pressure stable group.Statistics for each group respectively in each variable values,use statistical methods of mean of each variable between the three groups of statistical analysis,to whether there is a statistically significant blood pressure changes as the variable to hold the influence degree on the basis of comparing analysis.Results:In 3049 cases of hemodialysis,on the basis of dialysis related hypotension,dialysis associated hypertension diagnosis standard,the changes of blood pressure in the dialysis,results as follows: related hypotension patients occurred for 448 cases,accounting for 14.7% of all cases dialysis,related to hypertension patients for 245 times,accounting for 8.0% of all cases dialysis,blood pressure is stable in the rest of the dialysis,accounting for 77.3%.With hypotension group as the observation point view: the total ultrafiltration volume,standardized ultrafiltration volume,three variables such as the mean patient age were greater than high blood pressure and blood pressure stable group,have examined the statistical significant difference,and hemoglobin levels mean lower than high blood pressure and blood pressure stable group,but also in the statistical tests.Behind the front three variables is too large and the results indicate a variable is too small,can cause low blood pressure is an important factor.With hypertension group as the observation point view: the total ultrafiltration volume,standardized ultrafiltration volume,patient age hemoglobin content of these four indicators and hypotension group compared to the first three under low blood pressure,and the last group is higher than the low blood pressure,statistical tests all have significant differences,which in turn proves the conclusion above.Compared with blood pressure stable group,and standardized ultrafiltration volume,patient age and hemoglobin content of statistics analysis have significant differences,two total ultrafiltration volume variables p values > 0.05,no statistical significance.Within each group and the mean differences were seen between groups and statistical differences,look from the trend,the longer the age of dialysis,more prone to low blood pressure;Dialysis hypotension occurred time is 3-4 hours all the time;The primary disease of high incidence of low blood pressure,diabetes patients with primary glomerular nephritis and essential hypertension are more likely to occur hypertension;In serious deficiency of performance,low blood pressure tend to be more obvious.Conclusion:From the results of the study can get the following conclusion:1.The incidence of dialysis related hypotension in maintenance hemodialysis treatment was 22.7%,the incidence of dialysis associated hypertension was 8.0%,the risk of blood pressure stability is 77.3%;2.Common variable in dialysis: total ultrafiltration volume and standardized ultrafiltration volume is too big,easy to cause low blood pressure.Patient age,the more prone to low blood pressure,and the younger the easy to cause high blood pressure.Dialysis age more long more prone to low blood pressure,hemoglobin content is lower,the more likely to lead to low blood pressure.In the primary disease,diabetic patients tend to be more low blood pressure,and primary glomerulonephritis tend to be more in high blood pressure or hypertension.The more severe in deficiency syndrome,qi deficiency,the trend of hypotension occurred more obvious. |
