With the magnetic resonance(MR)technology continues to develop and progress,it has become the first choice for doctors in the diagnosis of knee disease.In the military training,the incidence of knee injury is high,it often lead to bone,meniscus,ligament,patellar fat pad damage,so it is reasonable by the MR to the knee military training injury research.On the other hand,the development of MR functional imaging technology has brought new ideas to the diagnosis of knee nerve injury.3.0T magnetic resonance new technologies-diffusion tensor imaging(DTI)technology and diffusion tensor tractography technology(DTT)have been widely used in the diagnosis of central nervous system diseases,and in the peripheral nerve.The application of injury remains to be further studied in the knee tibial nerve injury research,the domestic is no relevant exploration.In this study,we collected the MR test results of all the soldiers in our hospital from June 2014 to December 2015.After the two chief physicians jointly analyzed the summary opinion,the doctor finally concluded the diagnosis conclusion.First,the positive rate of knee injury was calculated,and then the distribution of various injuries in positive patients was analyzed.The overall situation was summarized,collated and analyzed according to the bone contusion,meniscus injury,ligament injury and patellar fat pad injury.In addition,one of the common causes of knee knee pain-patellar fat pad injury was classified and related analysis,in order to further strengthen the diagnosis and timely diagnosisand treatment of the disease.For patients with clinically diagnosed tibial nerve injury,routine scan sequence and DTI sequence imaging were used as the injury group.Plasma matched anisotropy(FA)and apparent diffusion coefficient(ADC)were measured by matching the same healthy volunteers as the control group.The values of FA and ADC were statistically analyzed by independent sample t test.P <0.05 was statistically significant,and the ROC curve of FA and ADC values was further plotted to analyze the specificity and sensitivity.Finally,Application of DTT showed normal and different degree of damage to the tibial nerve fiber bundle.The results showed that there were a total of 823 knee cases,left:340,right: 373,double: 55;male 745 cases,female 23 cases were treated with knee MRI in our hospital from June 2014 to December 2015,The age of 18-45 years,the average age of 26.7 years,a total of 756 knee injury,the positive rate of 91.9%,including bone contusion: 247 cases,398 cases of meniscal injury,ligament injury in 297 cases,107 cases of tendon and soft tissue injury,105 cases of patellar fat pad injury,790 cases of articular effusion,29 cases of joint capsule and synovial lesions,15 cases of nerve injury,91 cases of cartilage injury.20 patients with tibial nerve injury were examined by DTI.Results 15 patients had tibial nerve injury,ADC value and FA value were measured.The ADC value of the injured group was 1.43±0.07x10-3mm2 / s,the ADC value of the control group was 1.35±0.10x10-3mm2 / s,the FA value of the injury group was 0.51±0.05,the FA value of the control group was 0.58±0.05,the difference between the two groups was statistically significant;and the FA area under the ROC curve was 0.884,ADC area under the ROC curve area was 0.724,both between 0.7 to 0.9,showing its diagnosis of tibial nerve injury has a certain value.The diagnostic value of FA was 0.57,the index was 1.667,the sensitivity and specificity were 0.733 and 0.934,thecritical point of ADC was 1.35,the index was 1.467,the sensitivity and specificity were: 0.867 and 0.600.In conclusion,the incidence of knee injury caused by military training is high.MR can accurately diagnose bone contusion,meniscus injury,ligament injury and patellar fat pad injury in military training,and the initial treatment of DTI combined with DTT Nerve injury to explore and found that FA values and ADC values can help quantify the analysis of tibial nerve injury,and then further analysis of the degree of damage,and FA value of the diagnostic efficiency than the ADC value.Therefore,on the one hand hope that MR can be more applied to the diagnosis of knee disease,on the other hand hope that MR’s new technology can be better and broader development,open up the diagnosis and treatment of new world.
