The Advantage Of The Forgotten Joint Score For The Follow-up Of Patients With Total Hip Replacement

Objectives: To verify the validity of the forgotten joint score (FJS), and compare its difference with the ceiling effect and the reactivity of traditional joint score tools. And to study advantages of FJS score applying in follow-up of post-total hip replacement for expanding application of FJS score in the follow-up of post-total hip replacement in China.Methods: A total of 51 patients with unilateral total hip replacement were collected from Department of Orthopedics of the Second Affiliated Hospital of Kunming Medical University from June 2014 to December 2015. The average age of them is 61.4-yearold. There were 29 males and the average age is 61.2-year old. And 22 patients were females, the average age is 61.7-year old. There were 18 cases undergone Total hip replacement in the left side and 33 cases in the right side. There were 41 cases suffered from osteonecrosis of the femeral head (ONFH) or hip osteoarthritis, 9 cases suffered from femoral neck fracture, and 1 case in rheumatoid arthritis.After six months and twelve months of post-operation, Harris hip score (HHS score), Oxford hip score (OHS score), FJS score were performed for all patients.Statistical software SPSS 18.0 was applied for data analysis. The validity of the FJS score was verified by comparing the correlation with FJS score, HHS score and OHS score. the ceiling effects of FJS score,HHS score and OHS score at 6 and 12 moths of post-operation were compared,and the reactivity changes of three scores from 6 months to 12 months.The influences of gender,age and disease spectrum on FJS score were verified.Results: FJS score was highly related with HHS score and OHS score, the results were r= 0.839 and r=0.835, respectively. At the 6th month after operation, the ceiling effect of FJS score was 0, HHS score was 19.60 %, and OHS score was 15.68%. At the 12th month after operation, the ceiling effect of FJS score was 13.72%, HHS score was 29.41%, and OHS score was 25.49%. At the 12th month after operation, ceiling effect of FJS score was about half of HHS score and OHS score, respectively. Six months to 12 months after operation, the reactivity of FJS score change was 0.71, and that of the HHS score changes was 0.35, OHS was 0.39. There were no significant differences between FJS score,HHS score and OHS score in males and females after 6 months and 12 months of post-operation. There were no significant differences in comparing FJS score, HHS score and OHS score betwee patients over and under 70-year old aftet 6 months and 12 months of post-operation. And ther were no significan differences in comparing FJS scores of femoral neck fracture patients and non-femoral neck fracture patients after 6 months and 12 months, however, there was significant difference in comparing HHS score and OHS score.Conclusions: FJS score is a kind of patient-reported outcomes system (PROs) for evaluating the joint status after joint replacement, of which reliability, validity and internal consistency has been verified by many experts and countries. Its potential subtraction setting covers previous complaints from patients, such as pain, stiffness and poor function, low activity range, etc. and even including psychological assessment, it pays more attention to patient's own feelings and taking patients as the center,which expresses the concept of modern medicine " It's really good when patients feeling good". Its items are simple and easy for patients' answering and convenient for doctors' following up. Compared with traditional joint scoring tools, its main advantage lies in its low ceiling effect and high reactivity, which can allow us to observe the difference of patients with high joint scores, and patients' long-term joint changes. All the mentioned advantages are convenient for the study of modern joint replacement, and more suitable for the "double-high" era which meets high level of modern medical treatment and patients' high demand for joint replacement.
