Objective:To further understand the characteristics of APL,to analyze the effect of arsenic containing Chinese medicine on the complete remission rate and long-term survival of APL,to observe the effect of arsenic in different treatment stages,to analyze the treatment and time of treatment of APL with arsenic as the main program,APL treatment program to provide the basis.Methods:110 cases of APL patients admitted in our hospital from February 1991 to February 1917 were studied.At the beginning of treatment and consolidation of patients with arsenic-based treatment;in the outer hospital by retinoic acid or combined chemotherapy induced remission after treatment of our hospital patients with arsenic-based consolidation therapy;relapse/refractory treatment of patients with arsenic Induced.Analyze and compare the clinical characteristics and long-term survival of all APL patients.The effects of different treatment regimens on the recurrence-free survival rate(RFS),event-free survival rate(EFS)and overall survival(OS)were analyzed by Kaplan-Meier curve using SPSS22.0 software.Results:110 cases of APL patients,primary accounted for 92.7%,1.8%of tumor complications,psoriasis complicated by 5.5%.Early treatment of APL patients induced CR rate of 100%.In the induction phase,the RFS rate was 97.5%in the group of patients treated with arsenic,which was significantly higher than that in the consolidation group 80.6%(P<0.05).The EFS rates were(94.8%vs 78.6%),(91.5%vs 78.6%)(P>0.05)and the OS rates were(94.8%vs 86.4%),(91.5%vs 83)6%)(P>0.05).Recurrence of APL patients induced CR rate was 87.5%,and 10 years OS rate was 34.1%.Survival>5 years and continued treatment in our hospital of 29 cases of newly diagnosed APL patients,consolidation treatment 2.5 months to 103 months,12 to 24 months in 6 cases,the average As2O3 dosage 5.7 ± 1.84 courses,chemotherapy 2.75 ± 0.75courses;25 to 36 months in 6 cases,the average As2O3 dosage of 7.44 ±1.02 courses,chemotherapy 3 ± 0.55 courses;37 to 48 months in 8 cases,the average AS2O3 dosage of 12.04 ± 2.17 courses,chemotherapy 3 ± 0.58 courses;49 to 60 months in 1 case,the use of As2O3 10 courses,chemotherapy 5 courses;>60 months in 3 cases,the average As2O3 dosage of 15.32 ± 6.61 courses,chemotherapy 4.5 ± 3.5 courses;4 cases of treatment<12 months.13.3%of patients with newly diagnosed APL had significant liver damage(ALT or AST>300u/L)and severe neurotoxicity in the induction process.A total of 2 patients with APL were treated with second tumor.Conclusions:(1)In the treatment of APL,early use of traditionnal Chinese medicine arsenic can significantly improve the rate of remission and reduce the recurrence rate.(2)The use of arsenic in the consolidation period began to increase the use of recurrent patients in the treatment period,and it was suggested that the arsenic-based induction scheme should be adopted in the induction period.(3)Although most patients with relapse can be relieved again,but the remission is short,long-term disease-free survival rate of about 1/3.(4)Arsenic-based treatment regimen still has an optimized space,and the near-long-term toxicity of arsenic remains to be concerned.
