| As the "Healthy China 2030" and "Healthy Aging" has been proposed,the concept of health has developed from a single physical health to a physiological and psychological health,then health problems of the elderly become the focus of social concern.At the same time,the elderly group is in the later stage of life,and the health related quality of life under the influence of no physical disease,mental health and social function has become the topic of scholars and social research.There is a large Chinese population living in the countryside,and elderly population in countryside is under the dual influence of decline in physiological function and aging.How the status of health related quality of life is,what factors will influence the health related quality of life,what are the relationships among these factors and how to interact with each other,are to be explored.Objective:It will focus on rural elderly,discuss the influence factors of health related quality of life,analysis on the mechanism of these factors,and put forward the management countermeasures of health related quality of life for rural elderly,to provide strong evidence for decision making for society,and then to enhance the overall health level of rural elderly.Method:Qualitative analysis method and quantitative analysis method were used in the study.(1)In qualitative analysis,9 rural elderly from Nanyang city in Henan Province were investigated by semi-structured interview.(2)In quantitative analysis,with Multistage stratified random sampling,820 rural elderly from Nanyang City in Henan Province were investigated with questionnaires including the life event scale,general self efficacy scale,pain disclosure index scale,the perceived social support scale and 8 scales of entry concise Health.SPSS19.0 statistical software was used to make the descriptive statistics and Pearson correlation analysis about five variables of life events,self-efficacy,self disclosure,social support and health related quality of life.The regression analysis was used to explore the factors of life events,self-efficacy,self disclosure,social support how to influence the health related quality of life.Moderation test was used to discuss the mechanism of social support among life events,and health related quality of life.Mediated moderator test was used to discuss the mechanism of social support among life events,self-efficacy,self disclosure and health related quality of life.Result:(1)Variation analysis showed that,for health related quality of life,there were some significant differences in age(F=41.73,p<0.05),education level(F=46.45,p<0.05),chronic disease(t=21.94,p<0.05),etc.(2)Pearson correlation analysis showed that,life events had significant negative correlations with self-efficacy,self disclosure,social support and health related quality of life among rural elderly(r=-0.37,-0.18,-0.57,-0.46,p<0.05);self-efficacy had significant positive correlations with self-disclosure,social support and health related quality of life(r=0.31,0.40,0.24,p<0.05);self-disclosure had significant positive correlations with social support and health related quality of life(r=0.53,0.23,p<0.05);and it had significant positive correlations between social support and health-related quality of life(r=0.32,p<0.05).(3)Regression analysis showed that,life events,self-efficacy,self-disclosure and social support had apparent prediction function to health related quality of life(t=-9.57,4.15,15.82,13.64,p<0.01).(4)Moderation test showed that the regression coefficient was significance on the multiplicative interaction terms of life event and social support to health related quality of life(?=-0.31,t=-8.56,p<0.01).(5)Mediated moderator test showed that,the regression coefficients were significance,on the multiplicative interaction terms of life event and social support to self-efficacy(?=-0.17,t=-4.04,p<0.01),on self-efficacy to health related quality of life(?=0.26,t=7.98,p<0.01),on the multiplicative interaction terms of life event and social support to self-disclosure(?=-0.48,t=-10.17,p<0.01),and on self-disclosure to health related quality of life??=0.33,t=12.78,p<0.01).(6)Simple effect test showed that,when social support at lower levels,it had some significantly negative prediction function on life events to self-efficacy(?=-0.81,t=-15.26,p<0.05),self-disclosure(??-0.78,t=-13.89,p<0.05)and health related quality of life(?=-0.82,t=-15.92,p<0.05);when social support at higher levels,it had no any significantly prediction function on life events to self-efficacy(?=0.16,t=1.47,p>0.05),self-disclosure(?=0.21,t=1.96,p>0.05)and health related quality of life(?=-0.02,t=-0.36,p>0.05).Conclusion:Conclusion:(1)Demographic characteristics(age,education level,chronic disease,etc.),life events,self-efficacy,self disclosure,social support and so on,are the main factors that affect the health related quality of life of rural elderly.(2)For rural elderly,the social support not only has a direct moderating effect on the impact of life events to health related quality of life,but also has a indirect moderating effect through the two mediating variables of self-efficacy and self-disclosure.Meanwhile,when social support at lower levels,it has some significant influences on life events to self-efficacy,self-disclosure and health related quality of life.It shows that social support is a key element.(3)To improve the health related quality of life of rural elderly,we need not only pay more attention to self-adjustment ability of elderly,but also reinforce the social support. |
