Purpose: ABI is a simple and convenient, noninvasive and widely-used screening method to screen the LEAD. However,since the vascular calcification causing the vessels stiff in diabetes,ABI testing results still remain unreliable.Currently in primary medical care institutions overseas, they would use IC risk score to evaluate LEAD patients. IC risk score is a part of Framingham heart study , which takes seven factors as predicted factors to evaluate systematically and to predict the probability of intermittent claudication in the next four years, including age, gender,serum cholesterol,hypertension, smoking history, diabetes, and coronary heart disease. It would also be applied to evaluate the lower limbs artery lesion by some foreign institution. This study is mainly to evaluate the value of ABI and IC scores in early screening LEAD of diabetics.Method: the study objects are patients of endocrinology and inpatient department from the No.1 hospital attached to Nanchang university, collecting and studying the medical history documents which cover several routine biochemical indicators such as demographic characteristics, weight, diabetes course,smoking history and blood glucose and cholesterol etc ,and measuring ABI , performing Doppler ultrasonography in lower extremities. Then using the Doppler ultrasonography results as the clinical endpoints events,this study analyzes the ABI results, IC scores, ultrasonography separately in spearman way.Result : the study collects 252 patients’medical history documents,and 236medical history books are collected which are about left lower extremity ABI and ultrasonography. The result shows that the left lower extremity ABI and the ultrasonography have negative correlation (r=-0.186, P < 0.01 ),; the number of medical history about right lower extremity ABI and right lower extremity ultrasonography are 238, and the right lower extremity ABI and the ultrasonography have negative correlation (r=-0.199,P<0.01,).IC and LEAD: 249 medical history books were collected which are about IC risk score and severity of left lower extremity ultrasonography. It shows that the IC and the severity of left lower extremity ultrasonography have positive correlation (r=0.25O, P<0.01). The number of medical history books about IC risk score and the severity of left lower extremity ultrasonography are 250 and it shows that the IC and the severity of right lower extremity ultrasonography have positive correlation (r=0.245, P<0.01).Analyse the predicitve value of IC and ABI in ROC way. And the AUC of IC are higher than that of ABI both in left lower extremity and right lower extremity.Conclusion: ABI and LEAD have negative correlation.IC risk score and LEAD have positive correlation.The predictive value of IC is better than the predicitive value of ABI. |
