Objective:Analyze the correlation between ratio of ADC values of Pca and DWI score of Pca in PI-RADS V2,and investigate the value of ratio of ADC values in quantitifying DWI scoring.Methods: Collect 45 patients with prostate cancer who underwent the scan of multiparameter magnetic resonance imaging(T2WI,DWI,DCE)between March 2015 to December 2016 in Wannan medical college,all patients were confirmed by ultrasound guided biopsy pathology.According to the place of puncture,A and B,the two readers score the DWI image respectively independently according to DWI grading standard of PI-RADS V2 for prostate cancer,measure each patient’s ratio of ADC values.Analyze the consistency between A reader and B reader regarding DWI score,ratio of ADC values.Judge the correlation between ratio the of ADC values of prostate cancer and the DWI scores of prostate cancer of every reader。ROC analyzes the AUC and diagnostic performance of ratio of ADC values in the diagnosis of DWI scores ≥4.Results: 1.DWI scores result between A and B reader was highly consistent(weighted kappa = 0.691;95% CI,0.563-0.818).2.The ratio of ADC values between A and B reader was highly correlated(CCC =0.877;95% CI,0.787-0.930).3.For A reader,The DWI scores and ratio of ADC values were highly correlated(rho =0.852;95% CI = 0.747-0.917).For B reader,The DWI scores and ratio of ADC values were highly correlated(rho = 0.783,95% CI = 0.636–0.876).4.For A reader,the AUC the ratio of ADC values judging DWI ≥4 can is 0.965(95%CI,0.863-0.997),the sensitivity and speciality rate are 88.24% and 88.24%respectively.For B reader,the AUC the ratio of ADC values judging DWI ≥4 can is0.966(95% CI,0.864-0.997),the sensitivity and speciality rate are 93.94% and 91.67%respectively.For A and B reader,the diagnosis threshold of ratio of ADC values for judging DWI≥4 is >1.4.Conclusion:1.The results of DWI scoring and of ADC values measuring have good repeatability.Ratio of ADC values of prostate cancer and DWI scores of PI-RADS V2 of prostate cancer are significantly positively related.2.The ratio of ADC values can offset the deficiency of qualitative picture analzing to certain degree,and promote the stablity of DWI scoring,it can help the DWI scoring.Objective: Explore the correlation between related parameters of DWI and DCE and pathological Gleason scores prostate cancer.Methods: Select 45 patients with prostate cancer who were confirmed by pathology between March 2015 to December 2016,all patients have been socred by Gleason.All patients have underwent multi-MRI(T2WI,DWI,DCE)scan,according to b =1500 /mm~2,fit he ADC map,refering to biopsy site and abnormal performance of multiple parameters on magnetic resonance imaging,place ROI on the ADC map,get Pca related DWI parameters:ADC values,ratio of ADC values.And get DCE related parametersby Omni Kinetic post-processing software: Ktrans,Kep.All the parameters are obtained by A 、 B reader,the results are the mean of A 、 B reader.analyze relevance between DWI(ADC values,ratio of ADC values),DCE(Ktrans,Kep)related parameters and pathology Gleason score,prostate cancer will be in accordance with the Gleason score seperated into the high-risk group(Gleason score≥7),the low risk group(Gleason score≤6),DWI and DCE related parameters between the two groups using Welch-independent sample t-test,ROC curve analyze the AUC of DWI,DCE related parameters identifying the two groups.Results: 1.45 prostate cancer,the media-high risk group of lesions:a total of 32 lesions,a total of 13 low risk group.2.ADC values of prostate cancer and its pathologic Gleason scores showed a negative correlation,the correlation coefficient for rho is-0.568,ratio of ADC value and prostate cancer pathology Gleason scores were positively correlated,rho correlation coefficient was 0.663;Prostate cancer Ktrans value and prostate cancer pathology Gleason scores were positively correlated,correlation coefficient rho was 0.577,the prostate cancer Kep and prostate cancer pathology Gleason scores were positively correlated,rho correlation coefficients was 0.564.3.All prostate cancer focal ADC value was 0.770±0.233 mm/s,the media high risk group of cancer ADC value was 0.691±0.157mm~2/s,the low risk group cancer ADC value was 0.962±0.279mm~2/s.differences between the two groups was statistically significant(t’ = 3.286,P < 0.05);All the focal ratio of ADC values of prostate cancer was1.77±0.403,the media-high risk group,cancer ratio of ADC values was 1.912±0.381,low risk group of cancer,ratio of ADC values was 1.423±0.187,differences between the two groups was statistically significant(t’= 5801,P < 0.05).All prostate cancer focal Ktrans value was1.369±0.767 / min,the media-high risk group of cancer Ktrans value was1.626±0.751/ min,the low risk group cancer Ktrans value was0.737±0.306 / min,the differences between the two groups was statistically significant(t’=5.639,P < 0.05);All prostate cancer focal Kep value was 1.677±0.500/ min,the media-high risk group of cancer Kep value was1.848±0.483/min,the low risk group cancer Kep value was 1.256±0.212 / min,the differences between the two groups was statistically significant(t ’= 5.715,P < 0.05).4.ROC curve analysis indicates the AUC of DWI and DCE related parameters can identifying the two groups respectively were AUC(ADC values)= 0.865,AUC(ratio of ADC values)= 0.906,AUC(Ktrans)= 0.863,AUC(Kep)= 0.856.Conclusion: 1.Prostate cancer focal ADC values,ratio of ADC values,Ktrans values,Kep values have high correlation with pathologic Gleason scores and the highestcorrelation is ratio of ADC values,DWI and DCE related parameters can evaluate focal invasive prostate cancer.2.DWI and DCE related parameters(ADC values,ratio of ADC values,Ktrans,Kep)can preliminarily distinguish the risk of prostate cancer,but because the sample size is small,we can not give a definite diagnosis threshold,the need is to expand the content of samples. |
