Correlation Between TCM Constitution Of HBeAg Positive Chronic Hepatitis B And Treatment Response Of Peg-IFN A

Objective1.TCM constuitution of HBe Ag positive chronic hepatitis B for clinical prevention and treatment of this disease provide a basis for.2.To investigate the correlation between TCM constitution of HBe Ag positive chronic hepatitis B patients and treatment response of Peg-IFN? so as to explore the role of physical factors in the treatment of interferon.Methods This study includes two parts:1.In part one,one hundred and seventeen patients with HBe Ag positive chronic hepatitis B were collected from January 2014-December 2016,infectious ward or clinic of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine,devided them according to Wang Qi's classification for TCM constitution.2.In part two,ninety three patients with HBe Ag positive chronic hepatitis B who treated with Peg-IFN?were enrolled and devided into Qi deficiency group?yin deficiency group?qi-stagnationgroup?damp-heatgroup?yang deficiency group and other physical types group?At the end of the treatment,the therapeutic effect was evaluated by testing the level of ALT?HBe Ag and HBVDNA at 12^th week?24^th week and 48^th week respectively.Results1.Of the 117 HBe Ag-positive chronic hepatitis B patients,87 were male?74.4%?and 30 were female?25.6%?.There were 94 patients?80.3%?with the largest number of patients between 20 and 40 years of age.In TCM constitution,the proportion of Gentleness type accounted for 9.4%,partial constitution accounted for 90.6%.The constitutional types of the 117 patients from high to low rankings were wet-heat type,qi-depression type,qi-deficiency type,Yin-deficiency type,Yang-deficiency type,Phlegm-wetness type,Gentleness type,Blood-stasis type.In multiple pathology constitution,frequency of ranks from high to low were qi-depression type,qi-deficiency type,Phlegm-wetness type,Yang-deficiency type,wet-heat type,Yin-deficiency type,Gentleness type,Blood-stasis type.2.There were no significant differences in the age,sex,ALT level,HBVDNA level and HBs Ag level in each constitution groups before treatment.At the 12 th week of Peg IFN ? therapy,the ALT normalization rate of wet-heat group?qi-depression group and other group were higher in the patients,and the ALT normal rate of Yin-deficiency group was the lowest?P <0.05?.There was no significant difference in ALT normalization rate,HBVDNA negative conversion rate and HBe Ag seroconversion rate among groups at 24 th week?P <0.05?.At the end of the treatment,the HBe Ag seroconversion rate of qi-depression group was 68.4%,followed by wet-heat group?57.9%?,Yin-deficiency group is only 13.3%?P <0.05?.At the end of the treatment,there were significant differences in interferon response among groups?P<0.05?.Then in further comparison,when Yin deficiency group were compared with the wet-heat group and qi-depression group,P values were <0.05.Wet-heat group and qi-depression group is better than outher groups in response to interferon,and yin deficiency group had worst response.When HBe Ag-positive CHB patients with Yin deficiency or wet-heat are receiving interferon therapy,fever and flu-like symptoms appear more times?P<0.05?.In all patients,only 3 cases of depressive symptoms occurred in the qi stagnation group.Conclusion1.Most of HBe Ag-positive chronic hepatitis B patients were males in 30-50 years old.In TCM constitution of HBe Ag-positive chronic hepatitis B patients,wet-heat type,qi-depression type and qi-deficiency type accounted for the largest proportion.2.HBe Ag-positive chronic hepatitis B patients in wet-heat group and qi-depression group had better efficacy when treated interferon,and yin deficiency group had worst response.When HBe Ag-positive CHB patients with Yin deficiency or wet-heat are receiving interferon therapy,fever and flu-like symptoms appear more times.
