Surgical Option And Clinical Effect Of Adjacent Multilevel Spinal Tuberculosis

Objective: Through retrospective study to analyze the clinical effectiveness of three surgical method for anterior only,posterior only,combined posterior anterior approach;summarize the complication of different surgical approach and explore the best surgical approach for adjacent multilevel spinal tuberculosis.Methods: 41 adjacent multilevel spinal tuberculosis cases were collected and their clinical data were retrospectively analyzed that admit to first affiliated hospital of Xin Jiang Medical University between December2011 to December2016.According to the different surgical approaches,41 cases were divided into anterior only(group A),posterior only(group B),combined posterior anterior(group C)three groups.By observing A,B,C three groups of patients with neurologic improvement,total operation time,intraoperative blood loss,the total hospital stay days,TB outcomes and complications and radiographic measurement of correction rate,correction failure rate and bone fusion situation to compare the clinical curative effect of three surgical methods and took the statistical analysis.Results: Within each group pre and postoperative change of the Cobb angle,ESR,CRP was statistically difference.improvement of nerve function have statistically difference too.Main complications are surgical site infection,leakage of cerebrospinal fluid,fixation failure,recurrence of tuberculosis,etc.These complications are improved by relative management.Conclusion: Three surgical approach for anterior only,posterior only,combined posterior anterior approach all are effective,safety to treatment for adjacent multilevel spinal tuberculosis.Therefore,we should take into universal consideration to choose the best individualized operation method according to the patients clinical presentation and radiographic results,in this way,can we get the best curative effect.
