| Objective Analysis of the related risk factors and TCM syndromes of contrast induced nephropathy,in order to detect before or in the process of detection,as far as possible to reduce the risk factors,or in the early days of risk factors as possible intervention can improve the prognosis and recovery of renal function and to improve the survival rate of patients.Methods Through the investigation of the method of retrospective study,from January 2016 to December 2016,with Ultravist Omnipaque and ioversol angiography of the inpatients were 1424 cases,according to the three different contrast agents compared the incidence.And to select 478 cases of patients were examined using Ultravist specific research and analysis.Collect the clinical data of 478 patients,including the general situation,laboratory index,TCM syndrome,primary disease diagnosis,according to whether the occurrence of contrast nephropathy were divided into CIN group(21 cases)and non CIN group(457 cases),compared two groups in the differences between groups,correlation analysis of the indicators and occurrence contrast induced nephropathy.Results The results of this study showed that:(1)there were no significant differences in the effects of contrast media on the incidence of contrast induced nephropathy,the ratio was about 1:1:1.(2)Through the study of angiography in 478 cases of the use of explicit yourwill analysis found CKD incidence rate(23.8%)than in non CKD patients(3.4%)were significantly higher(P<0.05).(3)Compared with the non CIN group,the serum creatinine before and after CIN was significantly higher than that in the non CIN group(P<0.05)before and after the contrast group(CIN).The CIN patients were in the group of laboratory index comparison,CIN induced renal damage in the short term mainly for blood creatinine and urea were significantly higher(P<0.01),and the uric acid,albumin and hemoglobin were not significantly affected(P>0.05).(4)CIN group and non CIN group were compared.The study showed that coronary heart disease can influence the occurrence of CIN(P<0.05),the patients with hypertension and diabetes than the primary disease and the incidence of hypertension patients were significantly improved(P<0.05),but only in patients with hypertension or diabetes mellitus patients with the incidence of CIN had no obvious change(P>0.05).By two Logistic regression analysis,pure heart disease,hypertension and heart disease are the risk factors influencing the occurrence of CIN(P<0.05),with the OR value of 4.066(95%CI 1.273,12.988)and 3.888(95%CI 1.343,11.258).(5)CIN group and non CIN group were compared according to the TCM syndrome elements,and found that the incidence of damp syndrome and wind syndrome was higher than that of the single disease,and the incidence rate of blood stasis syndrome was higher than that of the deficiency syndrome(P<0.05).Conclusion In this study,the incidence of CIN was 4.4%in all the patients,and the incidence of CIN in patients with CKD was significantly higher than that in non CKD patients.The level of serum creatinine in patients with renal insufficiency is related to the occurrence of CIN,the higher the baseline creatinine in patients with CKD,the more prone to CIN.The acute renal injury induced by contrast media is mainly reflected in the serum creatinine and urea increased significantly in the short term,no significant effect on uric acid,hemoglobin,it can be speculated that urea can also be used as one of the early joint judging indicator for CIN.Coronary heart disease,hypertension and diabetes mellitus are risk factors for CIN.The incidence of hypertension and diabetes mellitus was significantly higher than that of patients without primary disease and simple hypertension.Wet syndrome and wind syndrome at the same time,the incidence of the disease was significantly higher than that of the individual,and the possibility of CIN was greater than that of deficiency syndrome. |
