| Objective:Through the cross-sectional investigation to know the current condition of diabetes distress and Uncertainty in illness among the type 2 diabetes patients,and to analyze their influencing factors.At the same inquiring into their relationship,so as to provide a theoretical basis for the health care colleagues to take more targeted nursing interventions that can improve the disease uncertainty in patients with type 2 diabetes,effectively reduce the diabetes distress in the future.Methods:From September to December in 2015,using convenient sampling to select 250 patients with type 2 diabetes in a grade 3 class A hospital in Dalian City.A cross-sectional survey was conducted by using self-designed patient general data questionnaire,Diabetes Distress Scale(DDS)developed by Polonsky,Uncertainty in illness Scale for adult(MUIS-A)developed by Mishel for the research.Using the SPSS17.0 statistical software for data input and analysis,using descriptive statistics(counting data using frequency,composition ratio described,measurement data using mean and standard deviation to describe),t test,single factor analysis of the variance,LSD comparison,Pearson correlation analysis was performed.Results:1.The total score of type 2 diabetes patients’ diabetes distress was(42.37±14.18)points,The average point of DDS was(2.49±0.83),four dimensions whose average score from high to low were: Emotional burden distress(3.28±1.39)points,Regimen-related distress(2.97±1.17)points,Interpersonal-related distress(1.76±0.96)points,and the Physician-related distress(1.45 ± 0.51)points;There was statistically difference of patients’ diabetes distress in terms of sexes,educational levels,occupations,average income per person in family of a month,Course of disease and Complication(P<0.05);However,the patients that with different age,marital status,Medical expense payment method,Current treatment modalities as well as Hb A1 c showed no significant difference with regard to the total scores of their diabetes distress(P>0.05).2.The total score of type 2 diabetes patients’ uncertainty in illness was(96.54±11.67)points,The average point of MUIS-A was(3.02±0.37),four dimensions whose score from high to low were: Uncertain accuracy(3.47±0.54)points,the unpredictability(3.05±0.54)points,Information deficiency(2.96±0.53)points,and complexity(2.22±0.40)points;Patients with low level of disease uncertainty accounted for 2.8%,moderate level accounted for 94.4%,high level accounted for 2.8%.Patients with different gender,age,education level,occupation,marital status,family per capita monthly income had significant difference(P<0.05);And patients with different Medical expense payment method,Course of disease,Current treatment modalities,Complication as well as Hb A1 c showed no significant difference with regard to the total scores of their uncertainty in illness(P>0.05).3.The type 2 diabetes patients with the diabetes distress and its four dimensions were positively correlated with the uncertainty of disease(P<0.01).Conclusion:1.Patiens’ diabetes distress score was at a moderate level,especially the Emotional burden distress and Regimen-related distress dimension score was highest,which should focus on strengthening the guidance and intervention of patients’ emotion and life;Diabetes distress scores of patients with different sexes,educational levels,occupations,average income per person in family of a month,Course of disease and Complication were quite different.2.The patients’ disease uncertainty score was above in a middle level,the Uncertain accuracy dimension score was relatively highest,which was supposed to be further reduced;uncertainty in illness scores of patients with different gender,age,education level,occupation,marital status,family per capita monthly income were quite different.3.Patients’ diabetes distress and uncertainty of disease were closely positive related,so in the clinical nursing work need to implement a targeted nursing strategies to reduce their uncertainty of disease,thereby improve the level of patients’ diabetes distress,improve the living quality. |
