Effects Of Secondary Warm Ischemia Time On The Prognosis Of Patients Following Liver Transplantation

Objective:Research about the effects of different SWIT on the prognosis of patients after liver transplantation,and help the surgeons choose a proper operation method.Method:Retrospectively analysed 109 cases who received liver transplantation at the first affiliated hospital of Zhejiang University during January 2011 to January 2015.And they were divided into three groups:group A(SWIT=0 minute),group B(0 minute<SWIT<30 minutes),group C(SWIT>30 minutes).We collected the indexes of liver function(include ALT,AST,ALP,DB,TB)before operation and post-operation.And the biliary complication,vascular complication,one-year patient’s survival rate and two-year patient’s survival rate are also considered.All of these data are analysed by the software of SPSS 20.0.And we use independent-simples T test to analyse measurement date,and chi-square test to analyse enumeration data,and those can’t be analysed by chi-square test,we choose Fisher exact test.All measurement date were expressed by mean ± standard deviation.A P value less than 0.05 was considered significant.Result:The peak of ALT of group A(1309.30±1220.58 U/L)and the peak of AST of group A(3419.34±3733.02 U/L)are significantly higher than the peak of ALT of group C(891.25±534.45 U/L)(P<0.05)and the peak of AST of group C(1693.71± 1210.76)(P<0.01).However,the indexes of liver function(include ALT,AST,ALP,DB,TB)have no obvious difference in all of these three groups 7 days after operation.The incidence of biliary tract complication of group C(7/24)and the incidence of vascular complication of group C(7/24)is significantly higher than the incidence of biliary tract complication of group A(7/76)(P<0.05)and the incidence of vascular complication of group A(8/76)(P<0.05).And the one-year patient’s survival rate and two-year patient’s survival rate have no difference in these three groups.Conclusion:The prolonged time of secondary warm ischemia(SWIT>30 minutes)will increase the incidence of biliary tract complication and vascular complication after liver transplantation.And those without secondary warm ischemia will elevate the short term liver transplantation,however,it doesn’t influence the recovery of liver function.And no matter if the patients have secondary warm ischemia or not,their survival rates don’t make a difference.
