ObjectiveTo explore the safety and feasibility of laparoscopic pancreatoduodenectomy in different levels of the center,to provide some reference for the promotion of pancreaticoduodenectomy.MethodsRetrospective analysis of September 2012 to September 2016 during the team in the Zhejiang University Shaw Shou Hospital and Zhejiang Provincial People’s Hospital and in the cooperative hospital consultation operation of 200 cases of laparoscopic pancreaticoduodenectomy patients Clinical data were divided into the hospital group(160 cases)and the external hospital group(40 cases).Patient factors include:1)general indicators:age,gender,BMI,with or without complications,with or without abdominal surgery history.2)intraoperative indicators:surgery time,intraoperative blood loss,whether to stay after the ICU and so on.3)postoperative recovery:get out of bed time,eating fluid time,whether surgery again,with or without postoperative complications,postoperative hospital stay.To explore the hospital group and outside the group between laparoscopic pancreaticoduodenectomy after short-term results were no difference.ResultsThere was no significant difference in gender,age and BMI between the two groups.There was no significant difference between the two groups in the preoperative complications.There was no significant difference between the two groups in the operation time(364.60min vs 379.00min,P>0.05).There was no significant difference in blood loss between the two groups(195.38 ml vs 225.68 ml,P>0.05).There was no significant difference in the complication rate between the two groups(40%vs 30%,P>0.05).There was no significant difference in the ICU rate between the two groups(13.1%vs 22.5%,P>0.05)No statistically significant.The first time of getting out of bed in this hospital group was shorter than that in the outside group(1.88d vs 2.95d,P<0.05),the difference was statistically significant.There was no significant difference between the two groups in the time of postoperative fluid intake(4.95d vs 5.35d,P>0.05).There was no significant difference between the two groups in postoperative hospital stay(17.61d vs 18.92d,P>0.05).There was no significant difference in the mean follow-up time between the two groups(30.4m vs 24.5m,P>0.05).ConclusionsCompared to OPD,patients can benefit from LPD,the promotion of LPD is necessary.In the full laparoscopic pancreaticoduodenectomy surgery experience and technical experts under the guidance of the promotion of LPD is safe and feasible.
