ObjectiveTo explore the characteristics of cognitive impairment in schizophrenia and compare the differences of olanzapine and aripiprazole on cognitive function in schizophrenia.Methods60 patients with schizophrenia(study group)were randomly divided into olanzapine group(30 cases)and aripiprazole group(30 cases),then they were given olanzapine or aripiprazole treatment for 6 weeks.Positive And Negative Syndrome Scale(PANSS),Wechsler Memory Scale-Digit Span(DS),Wisconsion Card Sorting Test(WCST),Iowa Gambling Task(IGT),Emotion Intensity Recognition Task(EIRT)were used to assess the patients’ mental state and cognition function before and after the treatment.At the same time,30 healthy people around the hospital were recruited into the control group and given the DS,WCST,IGT,and EIRT test.Try to explore the characteristics of cognitive impairment in schizophrenia by comparing the each field cognitive function of study group with the healthy group at baseline and find the difference of olanzapine and aripiprazole on cognitive function by comparing the each field cognition function of study group before and after treatment.Results1 The scores of PANSS after treatment of 6 weeks in olanzapine and aripiprazole groups were significantly decreased(P<0.05),but there were no difference between the two groups after treatment on the minus value of PANS S(P>0.05).2 Patients in the group of olanzapine and aripiprazole performed poorly than the healthy control at the scale score of DS(p<0.05).After treatment the two groups all got better scores at this period,but the group of aripiprazole got higher changed score than olanzapinere(P<0.05).3 Patients in the study group got higher total responses,error responses and lower classification numbers of WCST than the control group at baseline(p<0.05).After treatment,the group of olanzapine performed better at total responses,error responses and classification numbers of WCST while the group of aripiprazole got higher scores at classification numbers of WCST.There were no difference between the two groups of the changed value of WCST after treatment(P>0.05).4 Patients in the group of olanzapine and aripiprazole performed poorly than the healthy control at total score of IGT(p<0.05).After treatment,the total score and net score of per module of IGT were not different compared with that before treatment(p<0.05).And the changed value of all statistical indicators of IGT had no difference between two groups(p<0.05).5 The accuracy of six types of emotion in study group were significantly lower than the control group at baseline(P<0.05)except the fear and disgust emotion in the aripiprazole group.With non-positive facial stimuli,The patients provided higher intensity ratings on most non-target scales(P<0.05).After treatment,patients in the olanzapine group got higher accuracy of happy,fear,angry and disgust emotion(P<0.05)while the group of aripiprazole got higher accuracy of happy and sadness emotion(P<0.05).There were no difference on the changed value of all types emotion recognition accuracy between the two groups after treatment(P>0.05).6 The correct response,error response and classification numbers of WCST in patients with schizophrenia showed significantly correlation with the score of negative syndrome scale at baseline(P<0.05)while the net score of block 5 in IGT was positively correlated with the score of positive syndrome scale(P<0.05).After treatment,the patients’ changed values of WCST error responses,classification numbers and total score,net score of block 3 in IGT were correlated with the minus values of negative syndrome scale(P<0.05),the accuracy of disgust emotion was negatively correlated with the minus values of positive syndrome scale(P<0.05).conclusion1 The working memory,executive function,decision-making and facial emotion recognition of patients with schizophrenia are lower than the healthy people,With non-positive facial stimuli,patients with schizophrenia tend to have bias on emotion recognition.2 Olanzapine and aripiprazole can improve the mental symptoms and partial cognitive function of patients with schizophrenia.Two drugs have no difference on improving mental symptoms of patients but aripiprazole does better at improving working memory of patients with schizophrenia than olanzapine.3 The executive function,decision-making and facial emotion recognition are correlated with the positive and negative syndrome of patients with schizophrenia.
