| Objective: By using conventional occlusion paper, digital occlusal instrument for the purpose of establish a delay occlusal to adjustment occlusal.Comparing the results of changes in two methods of occlusal adjustment at once and within 6 months. To provide clinical reference for implant upper occlusion design. Methods:In 15 cases,on which implant single crown occlusal surface using 30μm occlusion paper to adjustment occlusal, for the purpose of establish delay occlusal.Using digital instrument for the other 15 cases occlusal adjustment in the purpose to establish delayed occlusion. After the end of the occlusal adjustment, using digital occlusal instrument to evaluation the success of the establishment of the delay occlusal, and record the occlusion data of immediately, first, third and sixth months after the implant restoration was completed of both groups.By comparing the occlusal data of the two groups after the end of the occlusal adjustment, selecting the most suitable method for the establishment for delayed occlusion. Results: 1.Simply use of occluding paper to occlusal adjustment in the purpose of establish delayed occlusion,only3 patients success;Contrast the occlusion data of using the paper and using digital instrument to adjust occlusal for the purpose to established the delay occlusal. Two groups implants occlusion data were compared, such as the area of implant occlusal force, the total set percentage, and the force ratio to the same name natural tooth, and the repair of dental arch and the contralateral side arch occlusal force ratio of full dentition, were statistically significant(P<0.05).2.Atthe first and third months after the completion of the occlusion,The occlusal data of the delayed occlusion which established by digital occlusal instrument have a significant difference between the results of occlusal adjustment by using occlusal paper(P<0.05).After first, third and sixth months, the occlusal force of the delayed occlusion group increased gradually, and the occlusal force of the repaired side and the healthy side of the arch was gradually balanced.Measurement the implant’s proximal and distal bone resorption at the first, third, sixth months time node after repaire completion. And through its own comparison of implant proximal and distal bone absorption without statistically significant difference(P>0.05);contrast the proximal and distal bone absorption of delay occlusion group and routine occlusion group in the three time points after the repair, the differences were statistically significant(P<0.05).3.After diffrent occlusal adjustment to two consecutive crown of implant, the nodes at the first, third and sixth months after the repair completion, proximal and distal bone resorption of each implants were compared, the difference was not statistically significant(P>0.05);in the same group gap of proximal and distal bone resorption also showed no significant difference(P>0.05);At first months after the repair, matching the gap of proximal end of the implant, the difference was statistically significant delayed occlusion group and routine occlusion group(P<0.05);with the the other time node, two groups respectively paired comparison, the implant bone absorption took place at the proximal and distal end,the difference was not statistically significant(P>0.05).Conclusion:1.The result of occlusal adjustment by usingocclusal paper was interfered many subjective factors, but the result of digital occlusal instrument was more objective and accurate.2 when the occlusion data at the time node of first and third months after the repair completion, there was significant difference between the two groups occlusal adjustment.Within 6months after the treatment, the occlusal force of the delayed occlusion group was increased gradually, and the occlusal force was gradually balanced. The delay occlusal for single implant, can reduce the implant proximal and distal bone resorption, can protect the repair package within six months.3.Delayed occlusion had no significant effect on the reduction of bone tissue around the implants of the two consecutive restorations which is retention by adhesives. |
