Objective: To investigate the expression of B7-H4 in different molecular subtypes of breast cancer and the regulation of CD8+T cells and CD68+ tumor associated macrophages in the immune microenvironmentMethods: From July 2014 to July 2016,70 patients with breast invasive ductal carcinoma confirmed by pathology were enrolled in the First Affiliated Hospital of Southwest Medical university.The related clinical and pathological data of patients were collected,including age,tumor size,lymph node metastasis,histological grade,WHO immunohistochemical results and molecular subtypes,clinical and pathological features of different molecular subtypes.Immunohistochemical method was used to detect the expression of B7-H4 in breast cancer tissues and the degree of invasion of CD8+T cells and CD68+ tumor associated macrophages in the tumor tissues.The expression of B7-H4 in 280 cases of breast cancer infiltrating ductal carcinoma cells with different molecular subtypes were compared,and the correlation between B7-H4 and CD8+T cells and CD68+ tumor associated macrophages was analyzed.Results: 1 the expression of B7-H4 in breast infiltrating ductal carcinoma with different molecular subtypes was different.The total positive expressionrate of B7-H4 in 280 cases of breast cancer was 67.9%(190/280),Luminal A positive rate was 60%(42/70),Luminal B positive rate was 55.7%(39/70),Her-2 amplification positive rate was 71.4(50/70),TNBC positive rate was 84.3(59/70),The difference between them was statistically significant.(?2 =15.785 P=0.001)2 B7-H4 was significantly correlated with histological grade and lymph node metastasis.(P<0.05),and there was no significant correlation between age and tumor size.(P>0.05)3 The infiltration of CD8+T cells and CD68+ tumor associated macrophages in different molecular subtypes of breast cancer were different in B7-H4 negative group and positive group.In Luminal A,B7-H4 negative group of CD8 + T cells in high infiltration rate was 78.6%,the positive group of CD8 + T cells in high infiltration rate was 23.8%,the difference is statistically significant(?2 = 20.302 P<0.001);B7-H4 negative group of CD68 + tumor associated macrophages in high infiltration rate was 32.1%,the positive group of CD68 + tumor associated macrophages in high infiltration rate was64.3%,the difference is statistically significant(?2 = 6.949 P= 0.008).In Luminal B,B7-H4 negative group of CD8 + T cells in high infiltration rate was 80.6%,the positive group of CD8 + T cells in high infiltration rate was35.9%,the difference is statistically significant(?2 = 8.022 P= 0.005);B7-H4 negative group of CD68 + tumor associated macrophages in high infiltration rate was 35.5%,the positive group of CD68 + tumor associatedmacrophages in high infiltration rate was 71.8%,the difference is statistically significant(?2 = 9.229 P= 0.002).In Her-2 amplification,B7-H4 negative group of CD8 + T cells in high infiltration rate was95.0%,the positive group of CD8 + T cells in high infiltration rate was 48%,the difference is statistically significant(?2 = 13.319 P<0.001);B7-H4 negative group of CD68 + tumor associated macrophages in high infiltration rate was 40.0%,the positive group of CD68 + tumor associated macrophages in high infiltration rate was78.0%,the difference is statistically significant(?2 = 9.351 P= 0.002).In TNBC,B7-H4 negative group of CD8 + T cells in high infiltration rate was100.0%,the positive group of CD8 + T cells in high infiltration rate was 49.2%,the difference is statistically significant(Fisher exact test P= 0.002);B7-H4 negative group of CD68 + tumor associated macrophages in high infiltration rate was 36.4%,the positive group of CD68 + tumor associated macrophages in high infiltration rate was74.6%,the difference is statistically significant(Fisher exact test P= 0.029).The infiltration of CD8+T cells in cancer foci was negatively correlated with B7-H4(r=-0.420 P<0.001),and The infiltration of CD68 + tumor associated macrophages in cancer foci was positively correlated with B7-H4(r=0.265 P<0.001),the difference is statistically significantConclusion: The expression of B7-H4 in different molecular subtypes of breast cancer is different,B7-H4 Correlate with lymph node metastasis and histological grade,And negatively correlated with CD8+T cells,positivelycorrelated with CD68+ tumor associated macrophages,B7-H4 in different molecular subtypes of breast cancer negative immune regulation.
