Objective:To investigate the degree of renal injury and prognosis in patients with lupus nephritis.Methods:303 cases of LN patients according to glomerular filtration efficiency level is divided into 3 groups,e GFR≤60m L/(min·1.73 ㎡)for 85 cases,e GFR﹥60~﹤90 m L/(min·1.73 ㎡)for 72 cases,and e GFR≥90 m L/(min·1.73 ㎡)for 146 cases;Recorded sex,age,24-hour proteinuria,hemoglobin,platelet count,antinuclear antibody ANA,anti ds-DNA antibody and complement C3,C4 level,mean arterial pressure,blood calcium,blood phosphorus,uric acid,SLEDAI score index,Univariate correlation analysis and multivariate regression analysis to assess the risk factors of renal injury.60 patients underwent nephropathy were divided into two groups according to the pathology type and clinical characteristics of the International Kidney Disease Association and the correlation with the independent risk factors.Results:3 groups of age,mean arterial pressure and 24-hour proteinuria,platelet count,hemoglobin,complement C3,serum uricacid levels were statistically significant differences(P ﹤ 0.05),Univariate correlation analysis showed that age(OR=1.31,95CI:1.685-0.711,P=0.000),low hemoglobin(OR=0.9,95CI:-0.032~-0.013,P=0.000),Low compelement(OR=0.34,95CI:-1.800~-0.338,P=0.004)high level of 24-hour proteinuria(OR=1.08,95CI:0.140 ~1.720,P=0.012),increasing of Uric acid(OR=1.02,95CI:0.004 ~ 0.008,P=0.000),low platelet count(OR=0.97,95CI:-0.050~-0.000,P=0.370)were the risk factors of renal injury.Multivariate logistic regression analysis showed that age(OR=1.67,95CI:0.024~0.078,P=0.000),anemia(OR=0.85,95CI:-0.029~-0.02,P=0.028),increased 24-hour proteinuria(OR=4.60,95CI:1.001~2.102,P=0.032),increased uric acid(OR=1.00,95CI: 0.005~0.002,P=0.000)level were independent risk factors for renal injury(P <0.05).the 60 LN patients who underwent nephrolithia,IV-type LN patients in 20 cases(33.3%),accounting for the highest proportion.the two groups of patients with pathological features,showing complement C3,complement C1 q,leukocyte infiltration,tubulointerstitial mononuclear Cell infiltration,tubulointerstitial fibrosis was statistically significant(P <0.05).Pathological type and 24-hour urine protein,uric acid,anemia,age and other indicators were positively correlated.Conclusion:Age,anemia,a large number of proteinuria and increased serum uric acid levels are related to furthur deterioration of renal function,and significance for prognosis.The prognosis of patients with LN has a certain correlation with the pathological type,also related with the general situation and laboratory indicators,However,at the clinical work,kidney wear has limitations,so this time according to the results of laboratory tests to determine the prognosis is poor or good,and guide the treatment.
