The Role Of EGFR In Predicting The Sensitivity Of Concurrent Chemoradiotherapy In Elderly Patients With Esophageal Cancer And Its Clinical Significance

Objective: To investigate the application value of epidermal growth factor receptor(growth factor receptor epidermal,EGFR)in predicting the sensitivity of radiotherapy and chemotherapy for elderly patients with esophageal cancer.Methods:Selection of our hospital in July 2014-July 2015 admitted during the period of a total of 60 patients with esophageal squamous cell cancer,male 56 cases,female 4 cases,aged 65-77.The location of the lesion was the lower thoracic segment。the lesion site lesions 2.5-8cm in length,TNM stage II-III period.Among them,20 cases of upper thoracic segment,accounting for 34%(20/60),in the middle of the chest in 23 cases,accounting for 38%(23/60),the lower thoracic segment in17 cases,accounting for 28%(17/60).II period of 30 cases,accounting for 50%(30/60),III stage in patients with a total of 30 cases,accounting for 50%(30/60).The pathological specimens of tumor tissues were obtained and the expression of EGFR was determined by mmunohistochemical staining 。 Radiotherapy for esophageal cancer: 6 mv-X irradiation in vitro,target dose of 45-60 gy,segmentation dose 2 gy/times,five times a/W。Paclitaxel and cisplatin combination chemotherapy(paclitaxel 135mg/m2 d1,cisplatin 20mg/m2 d1-5),every three weeks for a period of treatment,the total line synchronous 2 course of chemotherapy.Through After treatment,the results were divided into three levels:complete remission,partial remission and no remission.Finally,the relationship between the expression of EGFR and the remission rate of concurrent chemoradiotherapy was compared between the two groups.Results:according to the immunohistochemical evaluation method,60 cases of specimens can be divided into positive and negative 19 cases,41 cases were EGFR expression of EGFR expression positive specimens most cells of the cytoplasm and cell membrane dyed brown.EGFR expression of negative samples most cells did not color.EGFR positive group and complete remission rate is 28 cases,partial in 19 cases,4 cases were no relief;EGFR negative groups complete remission rate in 3patients,partial in 10 cases,6 cases no relief;EGFR positive group and complete remission rate(68.29%)is significantly higher than negative group(15.79%,P <0.05).Conclusion: If there is a high expression level of EGFR in patients with esophageal cancer,concurrent chemoradiotherapy can make patients get better clinical benefits in avoiding complications and injury caused by surgery.EGFR levels in patients with esophageal cancer may play an important role in predicting the sensitivity of concurrent chemoradiotherapy in elderly patients with esophageal cancer.
