Relationship Between HDL、UA And Vascular Endothelial Function In Patients With Coronary Heart Disease And The Correlation Between Them And The Severity Of Coronary Disease

ObjectiveTo investigate the relationship between HDL、UA and vascular endothelial function in patients with coronary heart disease and the correlation between them and the severity of coronary disease.MethodsA total of 243 suspected CHD patients were selected from April 2014 to October 2015,who were treated in cardiovascular medicine of Henan Province People’s Hospital.All patients underwent coronary angiography,including 146 cases CHD patients(CHD group)who were coronary angiography positive and coronary angiography negative in 97 cases non-CHD patients(non-CHD group).The above patients were treated with Endo-PAT2000 instrument for vascular endothelial function,computerized to obtain RHI(reactive hyperemia index).The two groups were compared the difference of characteristics、serum lipids levels、liver function、renal function 、UA and RHI;the correlative factors of vascular endothelial function(RHI)were analyzed using the Logistic regression model according to RHI normal or not;We investigate the linear relation between the HDL、UA and vascular endothelial function in CHD groups by using the Pearson correlation analysis;CHD patients were single-vessel disease group(37 cases),double-vessel disease group(48 cases)and triple-vessel disease group(61 cases)according to the severity of coronary disease,and the difference three group patients of HDL、UA and RHI were compared.Results1.The levels of HDL,vascular endothelial function evaluation index-RHI in the CHD group patients were significantly lower than those in non-CHD group patients[(0.99±0.21)mmol/L vs(1.08±0.24)mmol/L,(1.6±0.3)vs(1.8±0.5)](P<0.05).But the levels of UA in the CHD group patients were significantly higher than those in non-CHD group patients[(325.07±79.87)umol/l vs(321.52±85.50)umol/l](P<0.01).2.The association between UA and RHI was statistically significant in all cases(odds ratio was0.802,P=0.045).Blood pressure was associated with odds ratio was in non-CHD group(odds ratio was 0.979,P=0.005).The association between HDL、UA and RHI was statistically significant in CHD group(odds ratio was respectively 5.132 and 0.901,P value was respectively 0.043 and 0.004),and HDL and RHI was significantly positive correlation(r=0.470,P < 0.001),and UA and RHI was significantly negative correlation(r=-0.202,P=0.014).3.The level of HDL of three groups as the severity of coronary disease in CHD group patients were respectively(1.16±0.35)、(1.01±0.25)、(1.02±0.20);and the difference of groups was statistically significant(P=0.014),including HDL difference of single-vessel disease group and double-vessel disease and single-vessel disease group and triple-vessel disease group are statistically significant(P<0.05).4.The level of UA of three groups as the severity of coronary disease in CHD group patients were respectively(248.84±57.73)、(331.81±56.63)、(360.97±76.70);and the difference of groups was statistically significant(P<0.01),and the difference of any two groups are statistically significant(P<0.05).5.The level of RHI of three groups as the severity of coronary disease in CHD group patients were respectively(1.52±0.23)、(1.44±0.19)、(1.40±0.19);and the difference of groups was statistically significant(P=0.025),including RHI difference of single-vessel disease group and triple-vessel disease was statistically significant(P=0.007).Conclusion1.HDL is the protection factor of vascular endothelial function in CHD patients,HDL and vascular endothelial function were positively linear relation.However,the results of UA are contrary to HDL.Prompting that increasing HDL level and decreasing UA could may improve endothelial function,2.With the severity of coronary artery lesions,RHI gradually decreased,while the UA gradually increased.Suggesting that UA and vascular endothelial function are associated with the severity of coronary disease,which can predict the severity of coronary disease.
